无创机械通气对老年慢性阻塞性肺部疾病合并呼吸衰竭的治疗作用被引量：24

摘要目的评价鼻(面)罩无创双水平气道正压通气(BiPAP)在老年慢性阻塞性肺疾病(COPD)急性发作合并Ⅱ型呼吸衰竭的治疗作用。方法选择48例老年COPD合并Ⅱ型呼吸衰竭患者,分为常规治疗组23例,采用常规治疗;呼吸机治疗组25例,在常规治疗基础上加用BiPAP无创呼吸机治疗,观察两组患者心率、呼吸频率、血气分析、住院时间等指标。结果呼吸机治疗组患者心率,呼吸频率,动脉血气的改善优于常规治疗组,差异有统计学意义(P<0.05),且住院时间明显缩短,呼吸机治疗组无明显不良反应。结论无创双水平气道正压机械通气辅助治疗老年COPD合并Ⅱ型呼吸衰竭疗效确切,能快速缓解临床症状,提高PaO2、SaO2和降低PaCO2,纠正呼吸衰竭,缩短住院时间。
