摘要
目的观察高渗氯化钠羟乙基淀粉40注射液(HSH40)对内毒素性急性肺损伤(ALI)大鼠血流动力学和肺上皮屏障通透性的影响。方法健康雄性SD大鼠40只,随机分为4组,每组10只:①空白对照组(C组);②急性肺损伤组(L组);③HSH40组(H1组);④7.5%氯化钠高渗溶液组(HTS组,H2组)。H1、H2及L组静脉注射内毒素(LPS 5mg/kg)复制急性肺损伤模型,1m in后以5m l/kg的剂量分别输注HSH40、HTS及0.9%NS,C组用等量0.9%NS代替。观察输液前、输液结束即刻,2、4h时MAP的变化,测定输液结束4h后肺组织湿/干重(W/D)比和支气管肺泡灌洗液(BALF)中白蛋白含量。结果在输液结束2、4h,H1组的MAP明显高于H2组(P<0.01);在输液结束4h,H1组肺W/D和BALF中白蛋白含量明显低于H2组(P<0.01)。结论 HSH40治疗内毒素性ALI,维持血流动力学稳定时间较HTS更长;对肺上皮屏障通透性的改善作用优于HTS。
Objective To observe the effects of HSH40 on the hemodynamics and alveolar epithelium barrier of endotoxin-induced acute lung injury in rats.Methods Forty male SD rats were randomly divided into 4 groups with 10 animals in each group: group C,group L,group H1 and group H2.Lipopolysaccharide(LPS,5mg·kg^-1) was injected to induce ALI via femoral vein in group H1-H2 and group L.After 1 min,HSH40,HTS and 0.9%NS were continuously intravenous infused via femoral vein at a dose of 5ml·kg^-1 respectively;group C served as control one by 0.9%NS of the same volume.MAP was recorded before infusion,and 2,4h after infusion.The W/D lung weight ratio and the albumin concentration of BALF at 4h were detected after infusion.Results MAP of group H1 were significantly higher than that of group H2 at 2,4h after infusion(P〈0.01);The W/D lung weight ratio and the albumin concentration of BALF in group H1 were significantly lower than those of group H2 at 4h after infusion(P〈0.01).Conclusion The use of HSH40 for ALI is better than HTS in maintaining stable hemodynamics and improving permeability of alveolar epithelium barrier induced by LPS after resuscitation.
