摘要 目的:探讨手法复位结合HoffmannⅡ外固定支架固定治疗桡骨远端不稳定性骨折的手术技术和临床疗效。方法:回顾采用HoffmannⅡ外固定支架技术治疗的27例桡骨远端骨折患者(其中5例结合经皮克氏针固定)的临床资料,分析术后影像学指标和腕关节功能,进行疗效评价。结果:所有病例获得平均11.5个月(6~18个月)的随访,按Dienst功能评价标准优14例、良10例、可3例,优良率达88.9%。有5例病人出现影像学测量所见的复位丢失,有1例病人出现针道感染,无一例出现桡神经损伤和腕管综合征表现。结论:手法复位结合HoffmannⅡ外固定支架固定治疗桡骨远端骨折,操作简便,安全有效,是理想的治疗方法。 Objective: To explore the techniques and clinical effects of manipulative reduction and Hoffmann Ⅱ external fixation on unstable distal radius fractures. Methods: From June 2007 to March 2009, 27 cases of unstable distal radius fractures were treated by manipulative reduction and Hoffmann Ⅱ external fixation, with additional K-wires used in five cases. All the cases were followed up and evaluated according to the Dienst functional assessment system. Some radiographic parameters were also determined in this study. Results: All the patients were followed up for 6 to 18 months. 14 cases had excellent function, 10 better and 3 good according to the Dienst functional assessment system, the excellent and better rate being 88.9%. Among them, 5 had minimal loss of reduction, 1 had pin tract infection, and none had radial nerve injury or carpal tunnel syndrome. Conclusion: Combined with manipulative reduction techniques, Hoffmann Ⅱ external fixator is a convenient, safe and useful tool in treating distal radius fractures.
