摘要
目的探讨高原严重烧伤延迟复苏后早期大鼠脑组织病理变化及细胞凋亡的变化规律。方法130只雄性Wistar大鼠设计高原(海拔3800m)烧伤实验动物模型(TBSA30%,Ⅲ^0),并随机分4组:A组(高原烧伤即时复苏组)、B组(高原烧伤延迟复苏组)和C组(高原正常对照组),同时设D组(兰州地区正常对照组)(海拔1517m)。采用干、湿重比重法测定脑组织含水量,运用末端脱氧核苷酸介导的X—d UTP缺口末端标记法(TUNEL)测定脑皮质细胞凋亡阳性细胞数,光镜下观察病理变化。结果A组大鼠脑组织含水量于伤后6h开始升高,于24h达高峰(71.70±2.32),于伤后7d降至正常水平;B组变化趋势同A组。脑皮质TUNEL阳性细胞在伤后1h即增多,于伤后24h达高峰(233.33±16.22),于伤后7d仍高于正常水平。病理改变:C组脑实质内神经细胞、毛细血管形态基本正常。A组伤后早期(1h、6h)即见神经元嗜酸性染色增强,伤后24h病理变化明显,伤后7d基本恢复至正常;B组变化趋势同A组,伤后7d,神经细胞及血管形态、分布、周围间隙仍未完全恢复正常,部分神经元坏死、溶解。结论高原严重烧伤后,受伤大鼠迅速出现脑水肿,同时伴有脑皮质凋亡细胞显著增加,且延迟复苏组脑组织病理改变明显加重。
Objective To observe the dynamic changes of the apoptosis and pathology in the injured rats' brain tissues in earlier period after severe bum with delayed fluid resuscitation at high altitude. Methods At 3,800 meter above the sea level, the experimental bum animal model (TBSA 30%, Ⅲ^0) of high altitude was designed. One hundred and thirty mate Wistar rats were randomly divided into 4 groups equally: group A (delayed fluid resuscitation group), group B (immediate fluid resuscitation group), group C (normal control group at high altitude), and group D (normal control group at Lanzhou region 1517 meter). At different times after being burned, the brain water was determined by drying electro-thermostat and the apoptosis cells in pallium were detected by terminal deoxynucleotidyl transferase [TdT]-mediated deoxyuridinetriphate [dUTP] nick end-labeling (TUNEL) method. The pathomorphological changes in brain tissue were observed using hematoxylin and eosin (HE) stain method. Results The brain water contents in group A elevated on the early stage post burn in six hour, tended to the peak in 24 hours after being burned, and then gradually decreased. It returned to the normal level in seven days. The tendency of changes of brain water contents in group B was the same as that in group A, and it was abnormal in seven days after being burned. The number of apoptosis cells in pallium elevated in both group A and B in one hour after being burned, and reached the peak in 24 hours (233.67/high power field). The number of apoptosis cells in pallium in group A in seven days was higher than that in both of group C and D. The pathologic changes: the appearance of nearocytes and micranfium in rats' brain substance in group C was normal. In one hour and in six hours after being burned, the acidophilic stain of neuron accentuated and reached the peak in 24 hours, and recovered in seven days.
