摘要
目的探讨主动脉弓离断合并颅内动脉瘤的病因、影像诊断及处理方法。方法报道一例主动脉弓离断合并颅内动脉瘤的临床表现、影像资料及治疗过程,并结合文献讨论。结果主动脉弓离断合并颅内动脉瘤一般认为是由于主动脉弓离断致高血压脑病或由于侧支循环引起的颅内盗血而导致,是一种非常罕见的血管疾病,临床较易漏诊,确诊需行CTA或DSA成像检查。心脏畸形手术难度较大,颅内动脉瘤可行介入栓塞或手术夹闭治疗。结论主动脉弓离断合并颅内动脉瘤临床少见,必须提高对该疾病的认识,尽快确诊及治疗。
Objective The purpose of the study was to explore the etiopathogenisis,imaging diagnosis,operation technique of isolated aortic arch interruption（IAA）associated with intracranial aneurysm.Methods We report a 29-years-old patient with an isolated aortic arch interruption and a ruptured posterior communicating artery aneurysm and the related literature was reviewed.Results Intracranial aneurysm in cases of IAA was attributed to associated hypertensive encephalopathy or steal phenomena related to collateral flow.It was readily missed diagnosed.Multislice spiral CT and DSA can help to diagnose the rare vascular malformation.It was difficulty to treat IAA and the intracranial aneurysm can be treated with coil embolization or microsurgical clipping.Conclusions Intracranial aneurysm with IAA is a extremely rare malformation,It shoud be final diagnosed and treated as soon as possible.
