儿童精神分裂症患者认知功能与神经发育异常 被引量:6

Association between cognitive function and anomaly neurodevelopment in children with schizophrenia

摘要 目的:通过对儿童分裂症患者认知功能与脑室扩大及微小躯体异常关系的调查,探讨儿童精神分裂症认知缺损与神经发育异常的关系。方法:本研究设计为病例对照研究。选择70例新乡医学院二附院儿童少年精神科住院的首发儿童精神分裂症患者作为研究组,用躯体异常量表(Waldrop Scale,WS)进行微小躯体异常(minor physical anomalies,MPAs)评定,用CT测量脑室(脑室值用哈氏值、三脑室宽度、脑室指数、侧脑室体部指数、侧脑室宽度指数、前角指数表示),用《中国修订韦氏儿童智力量表手册》评定认知功能。选取对照组Ⅰ(n=42)与对照组Ⅱ(n=170),比较对照组Ⅰ和研究组之间的脑室值,对照组Ⅱ和研究组之间的MPAs,并进行认知功能与WS、脑室值之间的相关分析。结果:①研究组哈氏值[(5.28±0.37)vs.(4.94±0.34)]、三脑室宽度[(3.83±1.14)cm vs.(3.16±0.41)cm]及WS总分[(3.77±1.29)vs.(1.70±1.47)]均高于对照组(均P<0.01),脑室指数[(1.55±0.18)vs.(1.65±0.22)]及前角指数[(3.52±0.30)vs.(3.77±0.34)]低于对照组(均P<0.01)。②哈氏值与领悟、木块呈负相关(r=-0.26,-0.28;P<0.05),侧脑室体部指数和侧脑室宽度指数与图形排列呈正相关(r=0.33,0.32;P<0.01)。③WS总分与填图、操作总分及全量表分呈负相关(r=-0.29,-0.24,-0.25;均P<0.05)。④MPAs明显组(WS总分≥4)的算术[(8.29±2.72)vs.(9.63±2.31)]、木块[(5.21±2.53)vs.(7.25±2.70)]、操作总分[(31.53±7.35)vs.(35.34±8.04)]及全量表分[(74.29±10.23)vs.(79.94±12.76)]均低于MPAs不明显组(WS总分≤3)(均P<0.05)。结论:儿童精神分裂症患者认知缺损与脑室扩大和微小躯体异常有关,提示该病的认知缺损与神经发育异常有关。 Objective: To investigate the association of cognitive function to ventricnlar enlargement and minor physical anomalies ( MPA), and to explore the association between cognitive impairment and anomaly neurodevelopment in children with schizophrenia Methods: In this case-control study, minor physical anomalies, brain ventricles and cognitive function were assessed respectively with the Waldrop Scale (WS), brain computed tomography (CT) and Chinese Wechsler intelligence scale for children (C-WISC) in 70 first-episode childhood schizophrenia. The relation of C-WISC to WS and ventricular enlargement were analyzed. Results: ( 1 ) The patients had significantly higher Huckman' s value [ ( 5.28 ± 0. 37 ) vs. (4. 94 ± 0. 34 ) ], extend of third ventricle [ ( 3. 83 ±1.14) cm vs. (3.16±0.41) cm], WStotalscore [(3.77±1.29) vs. (1.70±1.47)], and lower indexes of ventricle [ (1.55±0.18) vs. (1.65±0.22)] and anterior angle [ (3.52±0.30) vs. (3.77±0.34)] than normal controls (Ps 〈0. 01 ) . (2) Huckman's value was significantly negatively correlated with scores of apperception and block design ( r = - 0. 26, - 0. 28 ; Ps 〈 0. 05 ) . Score of picture arrangement was significantly positively correlated with indexes of lateral ventricle body and extend of lateral ventricle ( r = 0. 33, 0. 32; Ps 〈 0. 01 ) . (3) WS total score was significantly negatively correlated with picture compltion and total scores of performance and intelligence test ( r = - 0. 29, - 0. 24 j - 0. 25 ; Ps 〈 0. 05 ) . ( 4 ) The patients with distinct MPAs (WS score≥4) had significantly lower score of arithmetic [ (8.29 ±2.72) vs. (9.63 ±2.31), P 〈 0. 05 ], block design [ ( 5.21 ± 2. 53 ) vs. ( 7. 25 ± 2. 70 ), P 〈 0.01 ], performance score [ ( 31.53±7.35 ) vs. ( 35.34 ±8.04), P 〈 0. 05 ] and total score of intelligence test [ ( 74. 29 ±10. 23 ) vs. ( 79. 94 ± 12. 76 ], P 〈 0. 05 ] than patients
