摘要
目的:探讨米非司酮在功能失调性子宫出血中的应用。方法:2008年1月～2009年10月门诊治疗的围绝经期功能失调性子宫出血129例采取米非司酮每晚睡前口服,每日1次,每次12.5mg,3个月为一疗程,观察治疗后月经情况,观察治疗前后血红蛋白、子宫体积、子宫内膜厚度并进行比较。结果:129例围绝经期功能性失调性子宫出血患者在治疗服药后均出现暂时性闭经。停药后30～50d内月经来潮49例,占总例数的37.98%,月经周期恢复正常,经量减少;月经紊乱表现消失71例,占总例数的55.03%,停药后直接进入绝经期;9例无效,占总例数的6.97%,因病情反复而行子宫切除术。本组病例采用小剂量米非司酮口服治疗总有效率为93.01%。治疗前与治疗后子宫体积比较,无显著性差异(P>0.05)。治疗前与治疗后子宫内膜厚度、血红蛋白比较,有显著性差异(P<0.05)。结论:小剂量米非司酮治疗围绝经期功能性失调性子宫出血临床疗效确切,应用方便,安全可靠,无明显的副作用,是作为治疗围绝经期功能失调性子宫出血的首选方法。
Objective：To explore the application of Mifepristone in dysfunctional uterine bleeding.Methods：From January 2008 to October 2009 in outpatient,129 cases of perimenopausal dysfunctional uterine bleeding with Mifepristone taken orally at bedtime,once daily and 12.5 mg every time,3 months as a course of treatment,the menstrual status after treatment were observed,the hemoglobin,uterine volume,endometrial thickness before and after treatment were observed and compared.Results：129 cases of perimenopausal dysfunctional uterine bleeding after taking treatment when patients suffered from temporary amenorrhea.30-50 days after stopping menstruation in 49 cases,with a proportion of 37.98%,menstrual cycles returned to normal by the amount of reduction in the performance of menstrual disorders disappear;71 cases with a proportion of 55.03%,after treatment directly into menopause;9 cases ineffective,with a proportion of 6.97%,while the line to be repeated condition hysterectomy.The total effective rate of patients with small dose of Mifepristone treatment was 93.01%.The uterine volume before and after treatment were compared,there was no significant difference（P0.05）.The hemoglobin,endometrial thickness before and after treatment were compared,there was a significant differ ence（P0.05）.Conclusion：Small dose of Mifepristone treatment of perimenopausal dysfunctional uterine bleeding has definite clinical effect,and convenient application,safe and reliable,no obvious side effects,it is the preferred method in the treatment of perimenopausal dysfunctional uterine bleeding.
