摘要
目的分析重症肌无力(MG)患者外周血单个核细胞(PBMC)胞质和胞核中糖皮质激素受体(GR)α和β表达与糖皮质激素(GC)疗效的关系,并探讨GC治疗MG疗效不同的机制。方法 收集24例MG患者和8名健康对照,根据GC疗效将MG患者分为GC敏感组、GC依赖组和GC抵抗组,每组各8例,应用Westernblot法检测MG患者GC治疗前后PBMC胞质和胞核蛋白GRtα和GRβ表达水平,并分析其与GC疗效的关系。结果正常情况下GRα主要表达于PBMC胞质中,GRβ主要表达于PBMC胞核中;GC抵抗组MG患者PBMC胞质中GRα表达水平降低(P<0.05),而GC依赖组MG患者PMBC胞核中GRβ表达增高(P<0.01)。GC治疗后,GC依赖组MG患者GRα的核移位率明显低于GC敏感组和GC抵抗组(P<0.01)。结论 GC抵抗型MG与其PBMC胞质中GRα低表达相关,GC依赖型MG与其PBMC胞核GRβ高表达相关,并可能与由此所致的GRα核转位降低有关。
Objective To detect the relation between the levels of glucocorticoid receptorα and β (GRa and GRβ) in peripheral blood mononuclear cells (PBMC) and therapeutic effect of glucocorticoid (GC) in myasthenia gravis (MG) patients, and to analyze the mechanism of various GC therapeutic effects. Methods Twenty-four MG patients and 8 healthy controls were enrolled in this study. In the MG patients, 8 were GC resistant, 8 GC dependent, and 8 GC sensitive. Total protein in cytoplasm and in nucleus in PBMC was extracted by NE-PER extraction kits, respectively. Antibodies specific to GRα or GRβ were used to detect the level of GRα or GRβ by the method of Western blot. Then the relationship between the GC therapeutic effects and the expression level of GR was analyzed. Results Normally, GRα was mainly located in cytoplasm, whereas GRβ in nucleus; The cytoplasmic GRα concentration in GC resistance group was lower than that in any of the other groups (P〈0. 05). Nuclear GRβ concentration in GC dependent group was higher than that in any of the other groups (P〈0.01). After GC treatment, GRα translocation rate in GC dependent group was lower than that in the other groups (P〈0.01). Conclusions GC resistant might be related to the low expression of GRα. GC dependent might be related to the high expression of GRβand the low translocation rate of GRα as well.
