摘要
目的分析婴幼儿心脏手术患者术中血糖变化情况,评价零平衡超滤对术中血糖水平的影响。方法回顾性分析70例婴幼儿患者的临床资料。根据术中是否使用零平衡超滤将患者分为超滤组和对照组。在体外循环前、阻断后、复温时、开放后、停机时、手术结束时抽动脉血监测血糖水平。利用重复测量的协方差分析评价零平衡超滤对术中血糖的影响。P<0.05为有统计学意义。结果血糖在体外循环开始后就开始升高,复温时达到高峰,并一直持续到手术结束。以平均血糖8.3 mmol/L为标准,41例患者发生术中高血糖。零平衡超滤对术中各时点血糖水平无明显影响。结论术中高血糖在婴幼儿心脏手术患者中比较常见。现有资料未能证实零平衡超滤有降低术中血糖的作用。
OBJECTIVE To describe variation in glucose level in infants receiving open heart surgery with cardiopulmonary bypass(CPB) and to evaluate the effect of zero-balanced ultrafiltration(ZBUF) on intra-operative glucose levels.METHODSThe clinical records of 70 consecutive infants receiving open heart surgery with CPB were retrospectively analyzed.Patients were divided into two groups according to whether the ZBUF technique was used during CPB.Intra-operative glucose levels were monitored at six different time points,as follows: before CPB,after aortic clamping,after rewarming,after aortic declamping,at the end of CPB and at the end of operation.Repeated measured analysis of variance was used to evaluate the effect of ZBUF on intra-operative glucose levels.A P value 0.05 was accepted as significant.RESULTS Glucose levels started to rise after the setup of bypass and peaked after rewarming and stayed unchanged during the rest part of the operation.Intra-operative hyperglycemia was common in infants receiving open heart surgery with CPB.Of the 70 patients,41 experienced hyperglycemia according to the diagnosis criteria of mean glucose above 8.3 mmol/L.The available data did not support that ZBUF had obvious impact on intra-operative glucose levels.CONCLUSION Hyperglycemia was common in infants receiving open heart surgery with CPB.We could not demonstrate that zero-balanced ultrafiltration ZBUF could decrease intra-operative glucose level.
