摘要
目的：评价采用肿瘤段桡骨切除,保留腓骨小头的自体腓骨移植治疗桡骨远端肿瘤的疗效。方法：行桡骨远端瘤段切除,用带血管自体近端腓骨移植,修复桡骨缺损并重建腕关节,术后就放射学及功能评价内容进行随访。结果：临床应用10例,术后平均随访29个月,局部复发1例,移植骨愈合良好,平均愈合时间为2.9个月。主观满意度为优良。重建腕关节活动范围掌屈21°~39°,背伸28°~57°,尺偏19°~41°,桡偏14°~19°。握力是健侧的37%~89%,无腕关节不稳和畸形。腕关节功能评价采用Enneking标准,优2例,良7例,一般1例。结论：桡骨远端骨肿瘤切除后,用自体腓骨移植治疗,既彻底切除了骨肿瘤,又保存了患侧上肢,同时较好地保留腕关节功能,是一种有效的方法。
Objective：To evaluate the use of bone tumor resection of the radial segment combined with autogenous fibula transplantation retaining a small head of fibula in treating distal radius tumor.Methods： We applied distal tumor resection with vascularized autogenous proximal fibular graft containing blood vessels to repair and rebuild the radial carpal bone defects and conducted follow-up evaluation after operation based on radiological and functional contents.Results：The method was clinically applied to 10 cases,with a mean follow-up 29 months.Local recurrence appeared in 1 case,grafted bone healed well,with an average healing time 2.9 months.Subjective degree of satisfaction was fine.Palmar flexion angle of the reconstructed wrist was 21°~39 degrees,dorsal extension 28°~57 degrees,ulnar deviation 19°~41 degrees,and radial deviation 14°~19degrees.Grip strength was unaffected,about 37% to 89% of the uninjured side,and there were no carpal instability or deformities.Functional assessment of wrist using Enneking criteria revealed excellent results in 2 cases,good in 7 cases,and fair in 1 case.Conclusion：Distal radius bone tumor resection with autogenous fibula transplantation can not only completely remove the bone tumor,but also save the ipsilateral upper extremity and reserve a good wrist function,thus is an effective method.
