摘要
目的:明确产超广谱β-内酰胺酶(ESBLs)大肠埃希菌基因型与耐药性的关系。方法:收集郑州大学第一附属医院、第三附属医院检验科细菌室2007年至2008年分离的产酶大肠埃希菌138株,采用PCR及DNA测序分析明确ESBLs型别及其分布模式,采用E-test法测定菌株的最小抑菌质量浓度(MIC)。结果:各菌株均含有1~5ESBLs基因型分布模式;不同模式菌株均对青霉素类、头孢一代、二代及三代中头孢噻肟和头孢曲松耐药(MIC≥256mg/L),对头孢他啶表现为体外试验敏感(MIC≤4mg/L),对四代头孢菌素头孢吡肟表现不一(MIC值分别为32、48、48、16和64mg/L)。对阿米卡星、亚胺培南及碳青酶烯类抗菌素稳定敏感(MIC分别为≤12和0.25mg/L)。携带不同ESBLs型别菌株抗药活性相同。结论:多种耐药机制可存在于同一产酶菌株中。
Aim:To research the relationship between extended spectrum β-lactamases(ESBLs) gene-type and drug resistence of ESBLs producing E.coli.Methods:A total of 138 ESBLs producing strains of E.coli were collected from the first and third teaching hospitals of Zhengzhou university.ESBLs genotype was detected by PCR and DNA sequencing.MICs of ESBLs producers were tested by E-test.Results:The resistant strains had 1~5 ESBLs genotype.Every strains was resistant to penicillins,the first and second generation cephalosporins,Cefotaxime and Ceftriaxone(MIC≥256 mg/L).All of them were sensitive to imipenem and amikacin(MIC≤12 and 0.25 mg/L),and every strain was sensitive to Ceftazidime in vitro(MIC≤4 mg/L).Different strain had a different result to cefepime(MIC was 32,48,48,16,and 64 mg/L),the drug resistence among 5 groups had no difference.Conclusion:Lots of drug-resistant mechanisms may exist in 1 ESBLs producing strains.
