摘要
目的 探讨Notch信号通路在膀胱移行细胞癌中的表达及与预后间的关系.方法 收集膀胱移行细胞癌组织新鲜标本70例,10例正常膀胱黏膜作为对照组.采用免疫组织化学、逆转录-聚合酶链反应(RT-PCR)和Western blot检测Notch-1、Notch-2、Notch-3、Jagged-1和DLL-1的表达.对70例患者进行随访,结合各基因表达分析其与术后生存时间的关系.结果 在10例正常膀胱移行上皮中以上5种Notch信号通路基因均为深染.非浸润性膀胱癌组织中染色主要集中于细胞膜上,染色较淡,浸润性膀胱癌中染色部位分布于细胞质中,呈片状浓聚.RT-PCR和Western blot发现5种因子在三种组织中的表达差异有统计学意义(P<0.05).非浸润性膀胱癌患者的术后无瘤生存时间与Notch-1和Jagged-1基因的表达状态相关(P<0.05).结论 在正常膀胱黏膜上皮、非浸润性膀胱癌和浸润性膀胱癌中Notch信号通路基因的表达呈现3种模式.非浸润性膀胱癌中Notch-1和Jagged-1基因的低表达与患者预后不良相关.
Objective To investigate the expression of Notch receptors and ligands in normal bladder transitional epithelium and transitional cell carcinoma of the bladder and its clinicopathological implications. Methods The expression of Notch-1, Notch-2, Notch-3, Jagged-1 and Delta-like-1 was detected respectively in 70 cases of bladder carcinoma, 10 cases of normal urothelia. Reverse transcription-polymerase chain reaction (RT-PCR) and Western blotting were used to assay the expression level of Notch factors. The predictive values of the expression for prognosis were investigated by Kaplan-Meier curve. Results All of the 5 kinds of Notch factors were intensively stained in normal bladder transitional epithelia immunohistochemically, but their expression was significantly decreased in tumor tissues. Moreover, the expression level of these 5 genes in the papillary tumors was lower than that in the invasive tumors. Postoperative disease-free survival time of patients with low expression of both Notch-1 and Jagged-1 was significantly shorter than that of patients with other expression patterns in papillary tumors (P 〈0. 05, respectively). Conclusion The expression pattern of Notch family in the papillary TCCB is different from that in invasive TCCB. Low-expression of both Notch-1 and Jagged-1 is potentially a useful marker for survival of patients with papillary TCCB.
