米力农在COPD并发急性呼吸衰竭机械通气中的应用

摘要 目的观察米力农对COPD并发急性呼吸衰竭机械通气患者脱机的影响。方法将COPD并发急性呼吸衰竭的机械通气患者62例随机分为两组,每组31例。2组病例均给予抗炎﹑祛痰﹑肠内营养支持﹑呼吸机参数调整,米力农组在此基础上加用米力农注射液0.5μg(/kg·min),微泵泵入10h,每日1次,应用时间为4~10d。结果米力农组在气管插管时间为(120±48)h,明显短于对照组的(212±72)h(P<0.05);米力农组二次插管病例数为3例(9.7%),明显少于对照组的9例(29%)(P<0.05);米力农组在24h内二氧化碳分压上升值为(11.6±3)mmHg,明显低于对照组的(19.7±4)mmHg(P<0.05)。结论 COPD并发急性呼吸衰竭的患者在机械通气过程中加用米力农可以缩短机械通气时间,改善预后。 Objective To observe the effect of Milrinone on mechanical ventilation time in COPD with lung function failure. Methods Sixty-two cases of COPD with lung function failure were divided into two groups at random:Milrinone group and control group with 31 in each group. Both groups were given routine antibiotic, expectorant, enteral nutrition,and ventilator parameter adjustment. 0.5μg/(kg·min) micro-pump intravenous Milrinone for 10h daily for 4 to 10 days was added to the Milrinone group. Results The ventilation time of the Milrinone group( 120 ± 48h) was shorter than that of the control group(212 ± 72h)(P〈0.05).The cases of reintubation in the Milrinone group(3/31,9.7%) were fewer than those of the control group (9/31,29%)(P〈0.05).The PaCO2 level of the Milrinone group( 11.6 ± 3mmHg) was lower than the control group(19.7 ± 4mmHg) in 24h(P〈0.05). Conclusion The addition of Milrinone in the mechanical ventilation can shorten the ventilation time and improve the prognosis in COPD with lung function failure.
