体外受精-胚胎移植周期中单卵双胎妊娠分析 被引量:5

The analysis of monozygotic twins in IVF-ETs

摘要 目的探讨体外受精-胚胎移植周期中单卵双胎发生率和相关原因。方法回顾性分析2006年1月至2009年5月间广州医学院第三附属医院生殖助孕科行常规体外受精-胚胎移植(IVF-ET)、卵胞浆内单精子显微注射术(ICSI)和冻融胚胎移植术(TET)后发生单卵双胎(MZT)患者的临床资料。统计MZT的发生率、流产率,分析常规IVF周期和ICSI周期、新鲜周期和冻融周期以及不同移植天数与单卵双胎妊娠发生率之间的关系。结果获临床妊娠1625个周期中,单卵双胎妊娠15例,发生率为0.92%,5例(33.3%)流产。比较单卵双胎组和非单卵双胎组患者年龄、不孕时间、促排时间、促性腺激素类似物(Gn)用量、获卵数、受精数和移植胚胎数,差异均无统计学意义(P>0.05)。常规IVF周期和ICSI周期、新鲜周期和冻融周期以及不同移植天数中MZT的发生率在各组间差异均无统计学意义(P>0.05)。结论体外受精-胚胎移植周期中MZT发生率高于自然周期妊娠。单纯的ICSI操作、冻融过程和移植天数与MZT的发生率的增加无紧密联系。 Objective To study the incidence and potential causes of monozygotic twinning(MZT) in IVF-ETs. Methods A retrospective study was performed on women with monozygotic twinning pregnancy in conventional in vitro fertilization and embryo transfer (IVF-ET) cycles, in intracytoplasmie sperm injection (ICSI) cycles and in frozen-thawing cy- cles in the Third Affiliated Hospital of Guangzhou Medical College between Jan. 2006 and May 2009. Incidence and miscarriage rate of MZT in IVF-ETs was investigated, and the potential relation of MZT was analyzed between in conventional IVF-ET cycles and in ICSI cycles, between in fresh-embryos transfer cycles and in frozen-thawing cycles and on different embryo-transfer day. Results Fifteen MZTs occurred in 1625 cycles with a incidence of 0. 92%, five of which developed miscarriage with rate of 33.3%. No significant difference was found between MZTs and other pregnancies in age, infertility time, days of Gn administration, ampules of Gn, the number of oocytes retrieved, the number of oocytes fertilizated and the number of embryos transferred. The incidence rate of MZT between in conventional IVF-ET cycles and in ICSI cycles, between in fresh-embryos transfer cycles and in frozen-thawing cycles and on different embryo-transfer day, was not statistical significance. Conclusion The incidence rate of MZTs in IVF-ET cycles greatly exceeds that seen following spontaneous conception. ICSI manipulation procedures, frozen-thawing procedures and embryo-transfer on different days are not close with an increased incidence of MZT.
