摘要
目的:观察捆绑带对动物模型骨折愈合过程中是否存在相关的影响因素,为临床应用提供依据。方法:30只新西兰大白兔随机分为两组,建立股骨干非负重骨折动物模型,对照组和实验组均采用髓内固定,实验组在骨折部位加用捆绑带捆绑固定,于术后2、4、6周行大体观察、骨密度测定、骨痂前后直径、组织学等检查进行比较,观察捆绑带在骨折固定中的作用及对骨折愈合的影响。结果:对照组在各观察时段各有1例出现骨折移位或畸形愈合。术后2、4周捆绑带组外骨痂相对少,两组在骨密度测定、骨痂前后直径无差别;术后4周捆绑带组光镜下新生骨小梁生长优于对照组,成骨细胞数多于对照组;术后6周对照组骨密度测定、骨痂前后直径明显大于捆绑带组,有明显统计学差异(t=4.243,15.909,P<0.05~0.01);光镜下捆绑带组骨痂较对照组显示更成熟并开始向板层骨转化,骨结构更致密,骨小梁排列更整齐。结论:在保持适宜的束缚力的情况下,捆绑带可以有效地维持骨折部位的稳定,避免出现骨折移位或畸形愈合,保障骨折正常的愈合,对于骨膜以及骨折部位血运没有异常的影响。
Objective: To observe the effect of cerclage band in the process of fracture healing applied and provide the basis for clinical application. Methods: 30 New Zealand white rabbits were randomly divided in two groups, the femoral shaft fracture animal model of non-weight-bearing were established, intramedullary fixation were used in control group and experimental group, the cerclage band was added at the fracture site for bundled fixed in the experimental group. In the two groups the gross observation, the bone mineral density ,the diameter of callus and histological examination were evaluated and compared at 2 weeks, 4 weeks, 6 weeks after operation, in order to understand the effects of cerclage band in fracture fixation and fracture healing. Results: In control group, each one cases of fracture displacement or malunion were found at all observation periods. At :2 weeks, 4 weeks, callus rarely appeared in the experimental group relative to the control group.The differences on diameter of callus and the bone mineral density between the control group and the experimental group were not significant (P〉0.05). At 4 weeks, under the microscope the growth of new trabecular bone in the experimental group were more significantly than that in the control group, the number of osteoblasts was more than that in the control group. The bone mineral density and diameter of callus in the control group were significantly increased than those in the experimental group after 6 weeks (t = 4.243, 15.909, P 〈0.05 ~ 0.01). The callus showed a more mature and begin to transform to lamellar bone, the structure of woven bone and the arrangement of bone trabecula were better in experimental group under the microscope. Conclusion: The cerclage band added at the fracture site can effectively maintain the stability, avoid fracture displacement or deformity healing and protect the normal fracture healing. The abnormal blood supply in the periosteum and the fracture is not found in cerclage band method.
