摘要
目的调查上海市杨浦区四平社区高血压患者一般状况与药物治疗状况,为社区高血压的防治提供相关信息和数据。方法对四平社区2004年1月1日～2009年5月1日建立高血压患者管理卡的5039例原发性高血压患者的相关信息进行统计、对照和分析。结果四平社区5年内全部管理在册的5039例高血压患者中,有临床并发症的患者占12.21%,缺血性卒中与心绞痛的发生率最高,分别为4.64%和2.34%,心血管高危与极高危者占41.32%,2级与3级高血压占74.38%。血压控制不良者占43.72%。高血压服药率为74.06%,其中85.21%的人服用1种药。常用的5种药物是珍菊降压片(32.02%)、氨氯地平片(27.33%)、硝苯地平片(15.38%)、复方降压片(10.77%)、氯沙坦(4.61%)。常用药物类型为钙拮抗剂(45.58%)、复方制剂(44.61%)、血管紧张素Ⅱ受体阻滞剂(8.82%)。结论上海市杨浦区四平社区高血压患者以2级与3级高血压及中高危患者占多数,血压控制欠佳,长效钙拮抗剂与血管紧张素Ⅱ受体阻滞剂的使用率明显提高。
Objective To investigate the general condition and treatment of patients with hypertension status from Siping community of Yangpu District in Shanghai,in order to provide relevant information and data for the prevention and control.Methods Siping communities hypertension management card 5 039 cases of patients with essential hypertension were related to information,analysis and statistics.Results 5 years Siping community management in the register of 5 039 cases of hypertension with clinical complications accounted for 12.21% of patients.Patients with high risk and very high risk accounted for 41.32%,2 level and 3 level of hypertension accounted for 74.38%.Poor control of blood pressure accounted for 43.72%.Patients with anti-hypertension drugs accounted for 74.06%,of which 85.21% of people taking a medicine.5 commonly used drugs Juzhen（32.02%）,amlodipine（27.33%）,nifedipine tablets（15.38%）,compound hypotensive tablet（10.77%）,losartan（4.61%）.Common type of medicine were calcium antagonist drugs（45.58%）,compound（44.61%）,angiotensin Ⅱ receptor blocker（8.82%）.Conclusion 2 and 3 level and high-risk patients with hypertension are majority in the Siping community.Poor blood pressure control and it is grim.Compared with the previous studies long-acting calcium antagonists and angiotensin Ⅱ receptor antagonist significantly increase the utilization of a smooth control of blood pressure and prevent complications obviously.
