同种异体骨在脊髓型颈椎病后路双开门椎管扩大植骨成形术中的应用 被引量:5

Application of bilateral open-door expansive laminoplasty with allogenic bone in treatment of cervical spondylotic myelopathy

摘要 目的研究同种异体骨在脊髓型颈椎病后路双开门椎管扩大植骨成形术中的价值。方法多节段脊髓型颈椎病患者20例,均行颈椎后路双开门椎管扩大植骨成形术,分别将3个同种异体骨块及2个术中切取的棘突骨块植入已打开的5个椎板间隙。术前及术后采用JOA评分标准评定患者脊髓神经功能状态,并对手术疗效进行评估。结果 20例患者中,术后12个月JOA评分比术前增加[(14.9±1.2)pointsvs.(9.8±0.8)points](P<0.01);术后各节段椎管矢状径皆超过14mm。术后6个月,同种异体骨完全融合及部分融合率低于自体骨融合率(53.3%vs.85.0%)(P<0.01);术后12个月,两种骨融合率差异无统计学意义(81.7%vs.92.5%)(P>0.05)。结论同种异体骨在颈椎管扩大植骨成形术中能够取得满意的疗效和植骨融合率,可以替代自身作为一种椎板间隔物。 Objective To study the value of allogenic bone spacers in posterior cervical bilateral open-door expansive laminoplasty for the treatment of cervical spondylotic myelopathy(CSM).Methods Twenty patients with multiple CSM underwent posterior cervical bilateral open-door expansive laminoplasty with allogenic bone spacers.C3-7 processus spinosus and vertebral plate were opened as double-door and implanted five bone spacers(three allogenic bones and two self-bones) randomly.The spinal nerve function was evaluated by JOA scoring system after operation and outcomes of operation were observed as well.Results The patients were followed up for an average time of 18.4 months.JOA scores were significantly higher on the 12th nonth than those before[(14.9±1.2) points vs. (9.8±0.8) points](P0.01).The mean sagittal diamenter of each segment was more than 14 mm 6 months after operation.The rate of hologamy and meromixis of allogenic bones was 53.3% and the fusion rate of self-bones was 85.0%(P0.05),which was not singificant on the 12th month after operation (81.7% vs. 92.5%)(P0.05).Conclusion Allogenic bone has a satisfactory curative effect and may be taken as a substitute for separating the vertibra.
