摘要
目的研究肾素血管紧张素(RAS)系统REN基因C-5312T、AGT基因A-20C和M235T、AGTR1基因A1166C、CYP11B2基因T-344C5种多态性与低肾素性高血压(LRH)的相关性。方法采用MALDI-TOF质谱检测技术对137例LRH患者和245例正常肾素高血压(NRH)患者进行5个基因多态性分型和相关性分析。结果发现LRH组较NRH组收缩压、血钠、24小时尿钠明显升高、血清高密度脂蛋白显著下降、左室重量指数明显增高。CYP11B2基因-344C等位基因频率在LRH组显著高于NRH(0.36vs0.26,P=0.003)。校正年龄、性别、体重指数后,-344C等位基因携带者LRH易感性增加(显性模式:OR=1.93,95%CI1.23-3.01,P=0.004),而其他基因多态性基因型和等位基因频率在两组间均无显著差异(P均>0.05)。CYP11B2基因C等位基因携带者与TT基因型相比,有更低的血浆肾素活性和尿醛固酮水平(P均<0.05)。结论 CYP11B2基因T-344C多态与LRH的易感性相关。
Objective To assess the relationship between 5 polymorphisms, REN gene C-5312T、AGT gene A-20C and M235T、 AGTR1gene A1166C、CYP11B2 gene T-344C and low renin hypertension (LRH). Methods MALDI-TOF analyses were performed to genotype in 137 LRH and 245 normal renin hypertensives (NRH). Results It was observed that LRH was associated with higher systolic blood pressure,higher level of sodium both in blood and urine, lower level of HDL and higher left ventricular mass index. The frequency of CYP11B2 the–344C allele was significantly higher in LRH group (0.36 vs 0.26,P=0.003). The C allele showed the significantly increased risk after adjusting for age, sex and body mass index (dominant model: odds ratio[OR]1.93, 95%confidence interval[CI]1.23 to 3.01, P=0.004). There was no signifi cant difference for other two 4 polymorphisms between LRH and NRH groups (all P〉 0.05). The CYP11B2-344C allele carriers had significant lower plasma renin activity and uric aldosterone level compared to T allele carriers (both P〉0.05). Conclusion CYP11B2 T-344C is associated with increased susceptibility to LRH.
