膝半月板及交叉韧带撕裂的磁共振扫描程序优化 被引量:10

Optimization of MRI Protocols in Examination of Patients with Cruciate Ligament and Meniscal Tears

摘要 目的:研究膝关节半月板及交叉韧带撕裂的磁共振(MRI)检查方法,优化扫描程序。方法:收集140例经关节镜证实的膝关节外伤病例,回顾性研究不同MRI扫描序列组(1个试验组和5个对照组)诊断半月板及韧带撕裂的准确性。结果:试验组诊断半月板和交叉韧带撕裂的准确性均大于其他对照组(P<0.05)。Osag-PDWIfs的诊断准确性较OsagT2WIfs好(N=192,P=0.000),CorPDWIfs明显优于CorT1WI(N=192,P=0.000)。试验组诊断内侧半月板撕裂准确性较外侧半月板好(N=192,P=0.000),诊断后交叉韧带撕裂准确性较前交叉韧带好(N=192,P=0.000)。结论:对于半月板及交叉韧带撕裂,本研究中试验组较其他试验组准确性好,为较优化的MRI序列组合,推荐其作为膝关节外伤的常规扫描方法。 Purpose: To optimize the MRI protocols in examination of patients with crucial ligament and/ or meniscal tears in knee. Methods: One hundred and forty cases of knee injury were divided into test group and control groups (the 1st - 5th) in this study. All cases had undergone arthroscopy, and the sensitivities, specificities and accuracies between the test group and different control groups were evaluated retrospectively. Results: The accuracy rate of test group with meniscal tears and crucial ligaments tears were obviously higher than all control groups(P 〈 0.05), the accuracy of the OsagPDWIfs was better than OsagT2WIfs(N = 192, P = 0. 000), and the accuracy of the CorPDWIfs was better than CorTIWI (N = 192, P = 0. 000). The accuracy rate of test group with media meniscal and posterior crucial ligament tears were higher than that of lateral meniscal and anterior crucial ligament tears. (N = 192, P = 0. 000) . Conclusion: In this study, the MRI sequences in test group are optimized protocols, which should be recommended to be used in routine clinical applications of the injured knee.
