摘要
目的总结股骨肿瘤切除后修复重建的经验教训。方法手术治疗16例股骨肿瘤患者,9例股骨近端肿瘤患者中7例采用瘤段切除+假体重建,2例股骨转子部肿瘤应用瘤段切除+Gamma钉+骨水泥固定;4例股骨干肿瘤采用瘤段切除+带锁髓内钉+骨水泥重建;3例股骨远端肿瘤采用瘤段切除+膝关节假体置换。结果 16例获随访,时间1年2个月～13年。按Enneking肢体肌肉骨骼肿瘤外科治疗重建术后功能评估标准进行评估:4～5分12例,3分3例,1分l例。结论严格掌握适应证、无瘤无菌操作、彻底灭活肿瘤截骨端是降低复发率的关键。有效的组织修复和功能重建不但可以保留股骨骨肿瘤切除后的肢体,而且具有良好的功能。
Objective To evaluate the effect of functional reconstruction after femoral tumor resection.Methods 16 cases were recruited.9 patients with tumor of the proximal femur were treated by segmental excision and endprosthetic reconstruction for 7 cases,and tumor of the femoral intertrochanteric region were treated by excision and Gamma interlocking intramedullary nail fixation and cement reconstruction for 2 cases.4 patients with tumor of the femoral diaphysis were treated by tumor segmental resection and interlocking intramedullary nail,3 patients with tumor of the distal femur malignant were treated by segmental excision and prosthetic replacement.All the prostheses were fixed with bone cement.Results All cases were followed up for 14 months to 13 years.All patients were in good condition during operation,whose joints were evaluated in regard to function with use of Enneking system,and 12 cases had 4～5 scores,3 got 3 scores,and 1 got 1 score.Conclusions It is key to decrease the recurrence with strict indication,non-neoplasm and aseptic technique,tumors astronomy plane should be deactivated thoroughly.Effective tissue repairing and functional reconstruction can not only reserve extremity,but also afford it fine function.
