摘要 目的编制适用于创伤性颅脑损伤(TBI)患者认知评定的计算机辅助系统并进行灵敏度和特异度的检测。方法在目前国内外已有的认知研究成果的基础上,结合TBI患者的临床表现,编制适合TBI患者认知评定的量表,运用计算机多媒体技术,将该量表的操作和评分过程转化成计算机操作软件,用于临床评估,并分析其灵敏度和特异度。结果生成计算机辅助认知测定系统,包括测评系统和测评管理系统两部分。结合约登指数得出TBI最佳截断点分值为41分,计算得出灵敏度为1,特异度为0.82。结论自行编制的计算机辅助认知评定系统操作简单、人机互动、即时反馈信息、数据存档和查询系统完善,以及评定结果准确、客观,并且具有良好的灵敏度和特异度,可以很好的运用于临床和科研,是一套较完善的计算机辅助认知评定系统。 Objective To provide the sensitive, objective and convenient means for determination of the cognitive function in the patients with traumatic brain injury (TBI). Methods A cognitive assessment scale suitable to patients with TBI was worked out on the basis of the achievement of the domestic and international research of cognition combined with the TBI patients' clinical manifestations. The processes of the operation and scoring of the scale were programmed into computer software by the computer multimedia technique. The sensitivity and specificity of the computer-assisted cognitive assessment system were compared with the clinical assessment. Results The generated computer-assisted cognitive assessment system included both the assessment and management systems. According to Youden's index, the optimum cut-off point score of TBI was 41 points, its sensitivity was 1 and specificity was 0.82. Conclusions Self-programmed the computer-assisted cognitive assessment system, which has some advantages including easy operation, convenient interaction between person and computer, instant feedback information, perfect data archives and query systems, correct and objective assessment, and good sensitivity and specificity, may applied to the be clinical and scientific research, and it is objective, convenient and sensitive mean of assessment of cognitive function in the patients with TBI.
