摘要
目的探讨37例小瞳孔白内障手术方法及疗效。方法虹膜后粘连患者用黏弹剂冲洗针头分离粘连;瞳孔膜闭、瞳孔闭锁患者用维钠剪做剪切分离,必要时行瞳孔缘多点放射状小切口。Ⅱ、Ⅲ级核行超声乳化吸除及人工晶体植入手术;Ⅳ、Ⅴ级核行小切口非超声乳化摘除及人工晶体植入术。结果术后37例患者视力均不同程度提高,术后3个月矫正视力≥0.5有26眼,2眼瞳孔直径>4.0 mm,均未见严重并发症。结论小瞳孔白内障患者依据具体病情制定扩瞳方法,并依据核的硬度决定采用超声乳化白内障摘除术或小切口非超声乳化白内障摘除术,术后能获得良好视力,无严重并发症。
Objective To investigate the surgery methods and clinical therapeutic effects in 37 small pupil cataract patients. Methods Used the ophthalmic viscoelastic agent and blunt syringe needle to separate the patients, posterior synechia , and used capsulotomy vannas scissors to shear the adhesive tissue of the patients who had corecleisis or atretopsia, level Ⅱ and Ⅲ lens nucleus patients used phacoemmulsification and intraocular lens implantation; level Ⅳ and Ⅴ lens nucleus patients used small incision non-phacoemulsification and intraoeular lens implantation. Results All 37 patients, postoperative visual acuities compared to that of the preoperative were improved at different degrees. The visual acuity was more than or equal to 0. 5 in 26 eyes and the pupil, s diameter was more than 4.0 mm in two eyes three months after surgery. There were no severe complications in 37 eyes. Conclusion The formulated method of expansing the pupil for small cataract patients will chose with different cases. According to the hardness of lens nucleus, phacoemulsification or small incision non-phaeoemulsification will be used. If we do the above, all patients will receive well vision, and have no severe complications.
