摘要
目的探讨在角膜穿孔伤修补术中应用角膜缘切口进行虹膜前后粘连分离,瞳孔区渗出物、前房内晶状体皮质、凝血块和积血清除术的手术效果。方法对45例(45眼)角膜穿孔伤术中应用角膜缘切口进行虹膜粘连分离,瞳孔区渗出膜、前房内晶状体皮质、积血和凝血块清除并前房注气。结果 45例中41例术后前后粘连及瞳孔膜闭发生率明显降低,为二期复明手术创造优良的条件,并术后保存较好的视力。结论角膜穿孔伤急诊行角膜全层修补术,术中应用角膜缘切口,进行虹膜前后粘连分离,瞳孔区渗出物、前房内晶状体皮质、凝血块和积血清除,治疗效果良好,为二期做视功能恢复手术创建了优良的条件。
Objective To discuss the effect of using limbus incision in corneal perforating injury surgery on iris syneehia separation and removal of exudate, lens cortex, clot and blood in anterior chamber. Methods We totally observed 45 cases （45 eyes） of corneal perforating injury. Limbus incision was done in the surgery for iris synechia separation, removal of membrane in pupil area, removal of cortex, clot and blood in anterior chamber, and inflation in anterior chamber. Results The incidence of iris syneehia and pupillary membrane were decreased, which provide better condition for secondary recovery surgery, and reserved better visual acuity. Conclusion Using limbus incision for separation of iris synechia and removal of anterior chamber exudate, clot and blood in emergency corneal repair surgery after perforating injury, will provide better condition for secondary recovery surgery.
