摘要
目的应用彩超显像技术探讨血栓闭塞性脉管炎(TAO)声像图改变及血流特征,比较X线血管造影与彩色多普勒血流显像在本病中的诊断价值。方法利用7.5MHz的彩超探头,探测51条病变的尺动脉、桡动脉、足背动脉血管和64条正常相应动脉血管,对其二维声像图及血流动力学指标进行了比较。结果TAO的血管壁增厚,>12mm(与正常组比较,P<0.05)。受累血管内膜面粗糙,可测及斑片样稍强回声。频谱峰值血流速度和舒张末期血流速度均有不同程度的增加,但以舒张末期血流速度增加更为显著,故阻力指数减低。但在9条显示纤细血流束的患者,其频谱为单峰型,血管阻力明显增高(RI>0.95)。CDFI的准确性及敏感性均高于周围动脉血管造影(PAG)(81.25%)。结论彩色多普勒血流显像可以对TAO进行定性、定量和血流动力学分析,且简便、可靠。
Objective\ To study the ultrasonographic changes and flow characteristic of thromboangitis obliterans(TAO) by color Doppler flow imaging(CDFI) and compare the diagnostic value between CDFI and periarteriography(PAG). Methods\ 51 pathological arteries (including radial, ulnar, dorsalis pedis arteries) and 64 normal arteries relatedly had been detected with 7.5 MHz color Doppler probe. TAO was compared with the contrast from twodimensional ultra sonograms and hemodynamics. Results\ T5BZThe walls of pathological vessels had thickened >12 mm( P <0.05). The spots of intenser echoes on the endangiums had been detected. The inner diameters of the vessels became narrow about 35% ̄49%. The value of peak flow velocities in systolic and diastolic final phases increased. The resistance indexes(RI) decreased, but the RI increased in 9 serious stenosis's vessels(RI>0.95). The accuracy and sensitivity of CDFI were significantly higher than those of PAG (81.25%). Conclusions\ The ultrasonograms and hemodynamics of TAO could be analysed by CDFI. CDFI can replace PAG as the first selective method in diagonsing of TAO.
