摘要
目的:探讨CT在骨肿瘤定性诊断中的价值。材料和方法:回顾性分析70例经手术、病理或临床证实的骨肿瘤CT资料,其中恶性肿瘤50例,良性肿瘤和肿瘤样病变20例,部分病例与平片资料进行对照。结果:在有平片资料的56例骨肿瘤中,平片定性正确38例,定性正确率679%,CT结合平片定性正确49例,定性正确率875%。CT定性的价值在于:①病变检出率高。②显示病变内部结构和骨外软组织肿块清晰。③显示病变的膨胀性和皮质完整性佳。④增强扫描利于确定病变的成份和血供。结论:CT对骨肿瘤定性有其独到之处,与X线平片相结合能明显提高定性正确率。
Objective:To evaluate CT in predicting the
nature of bone tumor.Materials and Methods:CT images of 70 patients with 50 malignant bone
tumors and 20 benign bone tumors or tumor-like lesions ,confirmed pathologically or clinically,
were analyzed retrospectively.The CT findings were compared with those from plain
radiography in regard to tumor detection and diagnosis.Results:Among 56 cases of bone tumor
with plain radiography,radiography correctly suggested a specific diagnosis in 38 patients and
the rate of the correct diagnosis was 679%.49 cases were correctly predicted with CT combined
radiography,and the rate was 875%.The merits of CT in predicting the nature of bone tumor were
as follows:The ability in detecting the lesion.Well demonstrated the component of the lesion
and soft tissue extension.Accurately depicted the expansion of the lesion and integrity of the
cortex.Soundly appreciated the histological component and vascularity of the lesion with
postcontrast scan.Conclusion:CT yielded new and unique information with combination of
radiography.CT can make a more accurate prediction of the nature of bone tumor.
