摘要
目的分析肺部炎性假瘤（pulmonary inflammatory pseudotumor,PIPT）的CT表现特点。方法回顾性分析21例经皮肺穿刺、胸腔镜及手术病理证实的PIPT患者的CT及临床资料。结果 21例中,单发者18例,多发者3例;27个病灶中,直径0.5~5 cm 23个,〉5 cm者4个;浅分叶征3个,桃尖征6个,边缘平直征7个,粗长毛刺4个,边缘光滑14个,呈圆形、不规则或片状。结论 PIPT的CT征象多样,鉴别诊断困难,全面分析其CT征象,结合临床资料有利于对本病的诊断与鉴别,诊断困难时应结合经皮穿刺或胸腔镜活检。
Objective To analysis CT features of pulmonary inflammatory pseudotumor.Methods We retrospectively analyzed the CT and clinical data of 21 patients with pulmonary inflammatory pseudotumor proved by percutaneous transthoracic needle biopsy,video-assisted thoracoscopic surgery and surgical pathology.Results The 21 cases included 18 cases of single focus and 3 cases of multiple focus.In 27 focuses,there were 23 ones with diameter of 0.5~5 cm,4 ones with diameter over 5cm.And there were 3 focuses with shallow lobulation sign,6 ones with peach point sign,7 ones with straight edge sign,4 ones with thick and long sentus as well as 14 ones with smooth edge.The focuses seemed round,irregular or like flakes.Conclusion Pulmonary inflammatory pseudotumor has multiple CT features.It is difficulty to carry out differential diagnosis.It is suggested to combine CT and clinical data to diagnose and differentiate pulmonary inflammatory pseudotumor.Percutaneous transthoracic needle biopsy and video-assisted thoracoscopic surgery can also be used when having difficulty to diagnose.
