枕下远外侧入路到达颈静脉孔区的骨性结构的显微解剖研究 被引量:2

摘要 目的:通过对骨性颈静脉孔(jugular foramen,JF)及毗邻结构显微解剖学特征的观察,寻求远外侧入路中可行的JF定位标志及显露方法。方法:手术显微镜对10例成人颅骨标本JF及其毗邻结构进行观察并测量相关的解剖学数据。结果:JF由颞骨和枕髁围成的不规则间隙,所有标本上均见到颞突,枕突只出现在40%的样本中;JF的大小和形态多变,多数样本的JF右侧大于左侧;JF前内侧有恒定存在的岩下窦沟,后外侧接乙状窦;JF内侧有颈静脉结节、内耳门;枕髁位于JF外口的下内方,形态多为肾形;JF外侧为茎突、茎乳孔及乳突。除JF外口的内缘与枕髁的最短距离以及茎乳孔与JF外口之间的距离以外,其余的关于JF内外口与毗邻结构之间的距离,在同一个体上的左右两侧无显著差异。结论:骨性JF与毗邻结构之间关系复杂。星点、茎突根部、乳突尖以及SS在体表的投影有助于处理JF区病变时准确定位。切除颈静脉突、乳突、颈静脉结节、及枕髁有助于JF的显露。
