摘要
目的：回顾性分析Ⅱ-Ⅳ型跟骨骨折钢板内固定手术后切口皮肤的情况,探讨预防发生术后皮肤坏死的方法.方法：2007年2月至2010年5月共收治46例患者,男36例（41足）,女10例（12足）;年龄16-69岁,平均41岁.其中高空坠落伤32例,交通伤10例,砸伤4例.7例双跟骨骨折,其中5例同时行双跟骨手术治疗,手术治疗跟骨共53足.根据sanders分型,Ⅱ型25足、Ⅲ型19足、Ⅳ型9足.行急诊手术治疗5例（6足）,受伤后至手术时间最长20d,平均8.16d,手术时肢体明显肿胀9例.结果：所有46例（53足）病例,3例（3足）术后出现切口皮缘坏死,占总病例数的5.66%,经加强换药后均在6周内痊愈,余病例术后切口愈合良好,拆线后均无皮肤坏死.结论：选择合适的手术时间,术中规范、微创的操作是避免跟骨骨折手术皮肤坏死并发症的有效途径.

According to sanders minute, Ⅱ 25 feet, Ⅲ 19 feet, Ⅳ 9 feet. The good emergency medical treatment surgery treats 5 examples （6 feet）, is injured late arrival surgery time longest 20d, average 8.16d, when the surgery body obvious swelling 9 examples. Results：All 46 examples （53 feet） case of illness,after 3 examples （3 feet） the technique appear cut the kou hides or fur reason necrosis,occupies the total case of illness number 5.66% ,trades the medicine after the enhancement to convalesce in 6 weeks,after - odd case of illness technique the margin cicatrization is good, after opens the line not to have the skin necrosis. Conclusion： Chooses the appropriate surgery time, the operation which in the technique the standard, micro creates is avoids the calcaneum bone fracture surgery skin necrosis complication the effective way.
