犬心肺复苏后血中游离脂肪酸、丙二醛和葡萄糖含量变化对脑复苏的影响

摘要 目的犬循环呼吸骤停18min复苏后观测8h血中游离脂肪酸(FFA)、丙二醛(MDA)和葡萄糖含量变化,同时观察神经机能恢复情况和骨骼肌症状。实验分3组即假手术组、常规复苏组和强力宁复苏组。结果①常规复苏组复苏期间FFA和MDA含量显著升高,强力宁复苏组则无明显升高;②两个复苏组血糖均于复苏后30min显著升高并达高峰,但于复苏2h后均迅速下降;③常规复苏组神经机能恢复明显慢于强力宁复苏组,而且震颤及或抽搐症状也有较高的发生率。结论心肺复苏后存在持续性高FFA血症和暂时性高糖血症,血中脂质过氧化物含量增加。血中FFA和脂质过氧化物含量增加,可能成为影响脑复苏效果的重要不利因素。复苏早期的暂时性高糖血症对脑复苏的预后究竟有多大的影响,尚难定论。 Objective The changes of blood free fatty acid (FFA),molondialdehyde(MDA)and glucose were observed during an 8-hour period of resuscitation following 18-minute circulatory arrest in dogs.Methods Among animals undergoing cardiopulmonary resuscitation,6 dogs were administrated intravenously with dexamethasone and mannitol,and other 6 dogs with Qianglining.Results The levels of FFA and MDA in blood were elevated significantly by ischemic danage and blood glucose levels obviously peaked in the period of 30 minutes following the begining of resuscitation but rapidly retored consequentially.Glucocortid and mannitol failed to give protective effects on the above changes during resuscitation.Intravenous Qianglining could keep blood FFA and MDA at lower levels during resuscitation.Conclusion A continuous increase of FFA and temproary hyperglycemin are present following resuscitation.The increase of blood peroxide and FFA may be a harmful factor to the outcome of resuscitation.It is hard to say how great does the temporary hyperglycemia affect the prognosis of cerebral resuscitation during early period of resuscitation.
