摘要
目的探讨不同病理类型胃息肉发生癌变的可能性及CyclinD1、p27kip1及幽门螺杆菌(HP)在不同病理类型胃息肉和胃癌中表达的相关性。方法采用S-P免疫组化方法检测CyclinD1、p27kip1在10例正常胃黏膜、20例慢性胃炎、20例增生型胃息肉、20例腺瘤型胃息肉、20例高分化胃癌中、20例低分化胃癌的表达,HP采用快速尿素酶法检测。结果 CyclinD1在增生型胃息肉和高分化胃癌中表达差异具有统计学意义(P<0.05)。p27kip1在正常胃黏膜和高分化胃癌中表达差异具有统计学意义(P<0.05);p27kip1在腺瘤型胃息肉和高分化胃癌中表达差异具有统计学意义(P<0.05);p27kip1在低分化胃癌和高分化胃癌中表达差异具有统计学意义(P<0.05)。HP在正常胃黏膜和高分化胃癌中表达差异具有统计学意义(P<0.05);HP在胃息肉与胃癌中阳性表达率差异具有统计学意义(P<0.05)。在胃息肉中CyclinD1和p27kip1两指标无等级相关性;在胃癌中CyclinD1和p27kip1两指标具有正相关性,相关系数Rm=0.5500,P<0.05。结论检测CyclinD1、p27kip1及HP可以作为诊断与评估胃息肉和胃癌发生、发展类型的主要指标。胃癌的发生可能与抑癌基因p27kip1的失活和原癌基因CyclinD1的激活有关。
Objective To explore the probability of different pathological types of gastric polyps to canceration,relation of helicobacter pylori（HP） and cyclinD1、p27^kip1 expression in gastric polyps and stomach neoplasm.Methods Immunohistochemical method of S-P was used to detect the expression of CyclinD1、 p27^kip1 in 10 cases of normal gastric mucosa,20 cases of chronic gastritis,20 cases of hyperplasy gastric polyp,20 cases of gastric adenoma polyps,20 cases of well-differentiated gastric cancer,20 cases of poorly differentiated gastric cancer.Helicobacter pylori（HP） were studied by method of rapid urea enzyme.Results There were statistical significance in Expression of CyclinD1 betwene hyperplastic gastric polyps and well-differentiated gastric carcinoma（P〈0.05）,p27^kip1 in normal mucosa versus well-differentiated gastric carcinoma（P〈0.05）,p27^kip1 in adenomas versus highly differentiated type of gastric polyps gastric carcinoma（P〈0.05）,p27^kip1 in poorly differentiated well-differentiated gastric cancer versus gastric carcinoma（P〈0.05）,HP highly differentiated in the normal gastric mucosa versus gastric carcinoma（P〈0.05）.Positive rates of HP in gastric polyp and gastric cancer had statistical significance（P〈0.05）.Two indicators of p27^kip1and CyclinD1 in gastric polyps had non-hierarchical relationship.Two indicators of CyclinD1 and p27^kip1 in gastric cancer had a positive correlation,the correlation coefficient rn was 0.5500,（P〈0.05）.Conclusion Detection of CyclinD1,p27^kip1 and HP can be used as diagnosis and assessment to the key indicators of gastric polyps and gastric cancer.Gastric cancer may be related to inactivation of tumor suppressor genes p27^kip1 and activation of proto-oncogene CyclinD1.
