摘要
目的探讨闭塞性脑梗死介入溶栓术的治疗与护理。方法通过采用1250mA-X线双臂数字减影心血管机,采用Seldinger技术,经皮穿刺股动脉,根据CT或MRI及临床体征定位,将5F～7F端孔多用途导管置入病变侧的颈内动脉内(尽可能接近栓塞部位),选用碘比乐或泛影葡胺,进行数字减影(DAS)脑血管造影,对32例急性闭塞性脑梗死患者进行介入溶栓治疗。结论介入性动脉溶栓治疗是一种安全、有效的治疗方法 ,选择好溶栓治疗适应症,认真做好术前准备、术后监控和护理(包括心理护理)个性化选择溶栓药物和剂量,能迅速有效的救治频临死亡的脑组织,从而有效降低致残绿,减少并发症,提高患者生活质量。
Objective To investigate the interventional thrombolysis of acute occlusive cerebral infarction and care.Methods With 1250mA-X ray digital subtraction cardiovascular machine,femoral artery was percutaneous punctured by Seldinger technique.According to CT or MRI and clinical signs of orientation,5F ~ 7F-side hole catheter was put into the target artery（embolization site as close as possible）,and cerebral angiography was done with iodine or diatrizoate.Results Thrombolytic therapy was performed in 32 patients with acute occlusion of cerebral artery.Conclusion Interventional arterial thrombolysis is a safe,effective treatment.Indication for thrombolytic therapy should be strictly chosen,and good preoperative preparation and postoperative monitoring and care（including psychological care） should be done.
