昆明地区儿童巨细胞病毒感染检测分析

摘要 目的联合运用Real—timePCR方法检测HCMV—DNA和ELlSA方法检测HCMV—IgM抗体,并分析HCMV感染实验室检测水平及患儿临床特征。方法收集2009年1月至2009年7月临床高度怀疑HCMV感染患儿300例,采集血清样本,分别用Real—timePCR检测HCMVDNA和ELISA方法检测HCMVIgM抗体。300例病例中有178例采集了尿液标本用Real—timePCR检测HCMVDNA。结果HCMV—DNA与HCMV—IgM阳性率差异显著,HCMV—DNA阳性率更高;尿液标本与血清标本HCMV—DNA阳性率差异显著,尿标本阳性率更高;小儿性别及年龄对HCMV—DNA感染无统计意义;HCMV引起的病毒性肺炎占所有病例的31.75%。结论临床应用两种以上方法检测HCMV感染有非常积极的意义;小儿年龄及性别对HCMV感染的影响没有差别;昆明地区小儿HCMV感染,病毒性肺炎为主要疾病。 Objective To detect HCMV-DNA via Real-time PCR and HCMV-IgM antibodies via ELISA method, So as to analyze the level of test HCMV infection in laboratory and clinical characteristics in children. Methods Collected 300 cases from January 2009 to July 2009, a high degree of suspicion of clinical HCMV infection in children, tested respectively with Real-time PCR detection of HCMV DNA and ELISA with detection of HCMV IgM antibodies. In these 300 cases, 178 cases of urine specimens collected and detected HCMV DNA by Real-time PCR. Results The positive rate of HCMV-DNA and HCMV-IgM in Plasma specimens was 21.00%, 16.00% respectively; The positive rate of HCMV-DNA were 33% and 55% in Plasma and urine specimens; There were non-statistically significant to infect HCMV at sex and age aspects. In all cases caused by HCMV viral pneumonia accounted for 31.75%. Conclusion It was necessary to use more than two Clinical applications to detect HCMV infection; there were no difference in children age and gender on the impact of HCMV infection. Viral pneumonia was the major diseases caused by HCMV infection in children in Kunming.
