摘要
目的:比较药物、高压氧、体外反博治疗突发性耳聋的疗效。方法:将136例(150耳)突发性耳聋分为3组:(Ⅰ)药物组46例(51耳);(Ⅱ)高压氧加药物组42例(46耳);(Ⅲ)体外反搏加药物组48例(53耳)。结果:(1)3组的治疗有效率分别为4118%、6957%和6792%。Ⅰ组与Ⅱ组或Ⅰ组与Ⅲ组比较,疗效差异有显著性(P<001),而Ⅱ组与Ⅲ组比较,疗效无显著性差异(P>005);(2)治疗效果与治疗是否及时呈正相关,其中7d内治疗有效率为9167%;(3)不伴眩晕者疗效好;(4)听力曲线类型为谷型、峰型、上升型者,疗效好;下降型或全聋者,疗效差;平坦型者居中。结论:(1)用高压氧或体外反博加药物来治疗突发性耳聋,其疗效均优于单纯使用药物;而比较高压氧与体外反博两方法,未能看出其疗效的差别。(2)治疗效果与治疗是否及时。
Aim: Sudden Deanfness can be cured with drug, Hyperbaric Oxygen, and External Counterpulsation, these treatments and their effective rates are compared.Methods:One hundred and thirty-six Patients were divided into three groups: (Ⅰ) treated with drug, (Ⅱ) drug plus Hyperbaric oxygen, (Ⅲ) drug plus External counterpulsation. Results: The average effect of three groups are 41.18% , 69 57%, and 67 92%, respectively. Results indicate that effect increases significantly if the patients are treated with Hyperbaric Oxygen or External Counterpulsation in addition to drug, however, there is no statistical difference between Group Ⅱ and Group Ⅲ, Other factors also affect the effect. First, time of treatment is critical to effect: treated soon the better. Treatments within 7 days of deafness resulted in hign effect as 91 67%. Second, vertigo syndrome is a bad sign: patients with vertigo syndrome have lower effect. Thirdly, audiometric curve is important: concave, convex, or ascending curve usually predicts higher effect than does flat curve; descending curve and total deafness are the worse cases.Conclusions: (1)The effect treated with Hyperbaric oxygen or External Counterpulsation in addition to drug is better than that treated with drug only. However, there is no statistical difference between Hyperbaric Oxygen and External Counterpulsation. (2) Three factors affect the effect: time of treatment, vertigo syndrome and audiometric curve.
