保留和不保留后交叉韧带全膝置换疗效的Meta分析 被引量:11

Posterior cruciate retained versus excised in total knee arthroplasty:a meta-analysis

摘要 [目的]比较全膝关节置换中保留和不保留后交叉韧带的临床结果,评价两种治疗选择的疗效差异。[方法]检索Medline(1966~2010.1)、Embase(1980~2010.1)、Cochrane library(2010.1)、CBM(1990~2010.1)等数据库,收集比较保留和不保留后交叉韧带全膝关节置换的随机对照试验,系统评价各研究方法学质量,并采用RevMan5.0.18进行Meta分析。[结果]纳入12个随机对照试验,共1 441膝,其中保留组697膝,不保留组744膝。组间比较术后半年至5年的临床疗效差别不大,Meta分析结果:膝评分、关节活动度、本体感觉和运动觉、并发症率等方面均无明显的统计学差异,但3个质量较高研究的关节活动度的统计权重明显偏向非保留组。[结论]目前证据表明膝关节置换中保留和不保留后交叉假体全膝关节置换的临床疗效差别无明显的统计学意义,但非保留组的关节活动度有大于保留组的倾向。建议开展更多高质量、大样本的随机对照试验以增加证据的强度。 [Objective]To compare the clinical effects between posterior cruciate retained and excised in total knee arthroplasty.[Method]The databases of Cochrane library(2010,Issue 1),Medline(1966-2010.1),Embase(1980-2010.1) and Chinese Biomedical Databases(1990-2010.1) were searched online.All the trials comparing posterior cruciate retained with excised for total knee replacement were included.Methodology quality of the trials was assessed and the data of the clinical outcomes in both groups were extracted.Revman 5.0.18 was used for data-analysis.[Result]In this study,the author included 12 randomized controlled trials,1441 cases were included(697 retained/744 excised),the result of Meta-analysis showed no difference in clinical outcome between the two groups,but the weight of the range of motion(ROM) in 3 higher methodological studies was favor of posterior cruciate excised group.[Conclusion]The ROM of the posterior cruciate excised may be higher than that of posterior cruciate retained at 5-year follow-up.More and stronger evidence is needed for supporting the outcomes.
