摘要
目的:探讨妊娠期糖尿病(GDM)及GDM并发妊娠期高血压疾病(PIH)患者血脂的变化。方法:回顾性分析502例GDM孕妇临床资料,按糖代谢异常程度分为妊娠期50 g葡萄糖筛查阳性168例、妊娠期糖耐量降低60例、妊娠期糖尿病274例,选择同期分娩的正常健康孕妇200例做对照,比较上述4组孕妇血脂水平。将GDM并发PIH程度不同分为正常血压的GDM、GDM并发妊娠期高血压、GDM并发子痫前期轻度、GDM并发子痫前期重度、GDM合并慢性高血压,比较上述5组血脂水平。结果:孕妇血甘油三酯水平随着糖代谢异常程度加重而显著升高(P<0.01),胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇水平随着糖尿病程度加重而显著下降(P<0.05)。糖代谢异常孕妇脂蛋白(a)水平显著高于糖代谢正常孕妇,而载脂蛋白a1、载脂蛋白b水平在4组间无明显差异(P>0.05)。GDM并发PIH孕妇仅甘油三酯水平明显高于正常血压组(P<0.05),其它血脂指标两组间无显著差异。高血压程度不同的糖尿病孕妇血脂水平无显著差异(P>0.05)。结论:GDM存在脂代谢紊乱,它与胰岛素抵抗有关联;早期改善脂代谢紊乱,对降低胰岛素抵抗、防治妊娠期糖尿病及其并发症具有一定意义。
Objective: To investigate the serum-lipid changes in gestational diabetes mellitus(GDM) patients with or without pregnancy induced hypertension(PIH). Methods: A retrospective study was conducted on 502 pregnant women with GDM who delivered their babies.Serum concentrations of lipids were compared according to different degrees of GDM and different severities of PIH.Results: Serum concentration of triglyceride(TG) elevated significantly in relation to the increased degree of abnormal glucose metabolism(P〈0.01) while TC,HDL,and LDL levels dropped(P〈0.05).Lipoprotein(a) levels in patients with GDM were significantly elevated.Only the level of TG in GDM patients with PIH was significantly higher than those without PIH(P〈0.05).Conclusion: Lipid metabolic disorders existed in women with GDM.Controlling lipid metabolic disorder earlier may improve insulin resistance and prevent GDM and PIH effectively.
