红细胞参数、红细胞脆性试验及G6PD活性检测在地中海贫血诊断中的应用 被引量:14

摘要 目的:探讨红细胞平均体积(MCV)、平均红细胞血红蛋白量(MCH)、红细胞体积分布宽度(RDW)、红细胞脆性试验以及葡萄糖6磷酸脱氢酶(G6PD)活性检测在地中海贫血(THal)在地中海贫血诊断中的应用价值。方法:健康对照组50例,地中海贫血患者135例,应用Sysmex 3000全自动血细胞分析仪检测红细胞MCV、MCH、RDW,用一管法测定红细胞脆性,紫外速率法测定G6PD活性,并对结果作相关统计学分析。同时将部分结果与地中海贫血基因诊断(PCR)结果进行比较,分析方法的灵敏度和特异度。结果:患者组MCV、MCH及红细胞脆性与正常对照组比较显著降低(P<0.01)。对照PCR结果,MCV、MCH、RDW、红细胞脆性试验的符合率分别为98.2%、87.9%、92.3%及85.7%。结论:MCV、RDW、红细胞脆性试验联合应用对地中海贫血有诊断价值,G6PD活性增高可作为地中海贫血的辅助诊断参考指标。 Objective: To investigate the value of the test of mean corpuscular volume(MCV),mean corpuscular bemoglobin(MCH),red blood cell distribution widh(RDW),erythrocyte fragility test and G6PD activity detection in diagnosing thalassaemia(THal).Methods: MCV, MCH,RDW were detected from 135 cases with thalassaemia.50 cases control by automatic blood analyzer system. G6PD activity were detected by rate method,and compared with the of gene diagnosis (PCR test)in order to evaluating their sensitivity and specificity. Results: The results of plasma MCV, MCH and Erythrocyte Fragility Test were significantly lower between the thalassaemia group and the control group (p〈 0.01). in contrast RDW and G6PD activity was significantly elevated (p〈0.01) .Conclusion : MCV, MCH and Erythrocyte Fragility Test could as significance level on screening thalassaemia in diagnosis. The test of MCV, MCH and Erythrocyte Fragility were very simple and effective in the prenatal screen of thalassemia, and the increase G6PD activity can be used in auxiliary diagnosis of thalassemia.
