摘要
[目的]探讨"液基薄层细胞学(TCT)+阴道镜+环状电圈切除术(LEEP)"阶梯式治疗在宫颈上皮内瘤变(CIN)中的应用及护理。[方法]对宫颈专科门诊行TCT机会性筛查病人2000例中结果异常的140例病人,行阴道镜下多点活检及LEEP术,对其结果进行回顾性分析与比较,总结治疗效果与护理经验。[结果]TCT检查具有高度的特异度及相对较低的灵敏度,阴道镜检查发现在不典型鳞状上皮细胞(AS-CUC)和鳞状上皮低度病变(LSIL)病人中存在一定比例的高度病变。经LEEP术后CIN的治愈率较高。[结论]"TCT+阴道镜+LEEP术"阶梯式诊治手段是目前治疗CIN、预防宫颈癌的有效方法之一。术前健康教育,TCT、阴道镜等检查的注意事项,LEEP术中的配合及术后的健康宣教对减少并发症及增加阶梯式治疗效果具有重要意义。
Objective：To probe into the application of ＂TCT＋Colposcope＋ LEEP（loop electrosurgical excision procedure）＂stepwise therapy for cervical intraepithelial neoplasi（CIN）patients and nursing care of them.Methods：A total of 140patients with abnormal results among 2000cases accepting TCT opportunity screening in cervical specialist out-patient clinic underwent multiple points biopsy under colposcope and LEEP.Re-sults of all cases were retrospectively analyzed and compared.Then cura-tive effect and nursing experience of them have been summarized.Results：TCT examination has a high level of specificity and relatively lower sensitivity.Certain proportion of patients with high lesions in ASCUC and LSIL patients could be found by colposcope examination.After LEEP operation,the healing rate of CIN patients was high.Conclusion：＂TCT＋colposcope＋ LEEP＂stepwise therapeutic approach is one of the most effective way for treatment of CIN and prevention of uterine cervix cancer.Preoperative health education,the announcements of TCT and colposcope examination,intraoperative cooperation of LEEP,and postoperative health propaganda have important significance for decrea-sing complications of surgery and increase the curative effect of stepwise therapy.
