摘要
目的分析老年人与中青年人消化性溃疡的临床症状、溃疡灶部位、幽门螺杆菌检测、并发症和伴发病,以提高对该病的诊治水平。方法选择经胃镜检查出的87例老年消化性溃疡患者作为观察组并与同期行胃镜检查的60岁以下的中青年消化性溃疡患者87例作为对照组进行对比分析,观察分析两组患者的不良影响因素、主要临床症状、溃疡灶部位、幽门螺杆菌检测、并发症和伴发病等。结果两组患者长期吸烟、饮酒、服用NSAIDs等不良影响的比例均居高;32例老年人临床症状不典型;胃溃疡的发生率观察组46例高于对照组22例;穿孔在中青年人多见,上消化道出血是老年人主要并发症之一;过半数以上老年人溃疡伴发三种及以上慢性疾病。结论老年人消化性溃疡的胃溃疡构成比高,并发症发生率高,因其临床症状不典型而易被忽视,合并全身性疾病而增加病死率,所以内镜下明确诊断和及时止血治疗为首选治疗。
Objective To analyze the elderly people and the Middle - aged or young people with peptic ulcer about clinical symptoms, ulcer lesion spot, the pylorus spiral bacillus examines, the complication and the partner morbidity, in order to elevate the level of diagnosis and treatment of this disease. Methods 87 old patients with peptic ulcer which were checked by the gastroscopy, were chosen as the observation group, and 87 cases with the same disease lower than 60 years old as the control group, observed and analyzed two groups about the adverse effect factors, the main clinical symptoms, ulcer lesion spot, examines of the pylorus spiral bacillus, the complication and the partner morbidity and so on. Results The ratio of long - term smoking, drinking more wine, to take NSAIDs in two groups were all high which induced the adverse effects; in 32 old patients the clinical symptoms are atypical; 46 cases of the gastric ulcer formation rate in the observation group is higher than 22 eases in the control group ; The perforation often happened in the middle - aged or young people, the upper gastrointestinal hemorrhage is one of the main complications of old patients; Over a half of old patients have three kind or more chronic diseases. Conclusion In old people peptic ulcer companied with gastric ulcer is high, the complication rate is high, too. Because of its atypical clinical symptoms, which is not easy to find out, and with the merge body -wide disease, the case fatality rate increases, so get the right diagnosis through the endoscope and stopping the hemorrhage are the best treatment way.
