摘要
目的:比较不同麻醉方法用于婴幼儿支气管异物取出术的效果,探讨针对该类患儿合适的麻醉方法。方法:选择因支气管异物需行异物取出术婴幼儿46例,随机分为A、B两组。其中A组23例,实施γ-羟基丁酸钠、氯胺酮和咪达唑仑静脉复合麻醉,术中保留患儿自主呼吸;B组23例,施行氯胺酮、异丙酚、维库溴铵静脉复合麻醉,术中打断患儿自主呼吸并予人工通气。观察并记录两组患儿麻醉时间、手术时间及苏醒时间,统计两组病例的置镜满意率及并发症发生率。结果:与A组比较,B组患者麻醉、手术及苏醒时间均明显缩短,置镜满意率明显提高(P<0.05),并发症发生率明显降低(P<0.05)。结论:在确保有效通气的前提下,维库溴铵、氯胺酮复合异丙酚麻醉方式用于婴幼儿呼吸道异物取出术是安全、有效的,具有缩短麻醉、手术及苏醒时间,提高置镜满意率以及降低并发症发生率的优势,是一种针对该类患儿的合适的麻醉方式。
Objective: To compare the different methods of anesthesia for infants and young children removal of bronchial foreign body effect,to explore appropriate methods of anesthesia. Method:Infant and child airway foreign body removal in 46 cases ,according to anesthesia is divided into two groups. Group A,for the γ-Hydroxybutrate group (23 cases),the implementation of γ-Hydroxybutrate,and the force of ketamine intravenous anesthesia on the West to preserve spontaneous breathing. Group B,propofol group (23 cases),the implementation of ketamine,propofol ,victoria bromide intravenous anesthesia ,artificial ventilation technique Bank. Two groups were observed and recorded children with anesthesia time ,surgery time and recovery time ,statistics the two groups of the satisfactory rate of micro-scope olaeing in the incidence of complications. Results..By compareing with group A,the time of anesthesia,surgery and recovery in group B were significantly shortened,the satisfactory rate of microscope plaeing was significantly higher than Group A(P〈0.05),complications were significantly lower than the Group A(P〈0.05). Conclusion:Under the premise of effective ventilation,combined anesthesia for infant with bronchial foreign body removal by ketamine ,propofol and vecuronium was safe and effective ,as a proper anesthesia for these patients ,which had the advantage of shorting the time of anesthesia,operation and analepsia,improving the satisfactory rate of microscope placing, and lowing the rate of complications.
