脉压差对高血压病人心脏结构和功能影响的超声研究 被引量:1

摘要 目的探讨高血压患者脉压差与左心室结构和功能改变的相关性。方法将277例高血压患者分为脉压差(PP)≥40mmHg的A组和20mmHg≤PP<40mmHg的B组,对这两组患者分别进行心脏超声检查,检测并记录舒张期室间隔厚度(IVSDd)、左室后壁厚度(LVPWDd)、左心室舒张期内径(LVDd)、收缩期内径(LVSd)、左心室收缩功能指标射血分数(LVEF)、左心室舒张功能指标二尖瓣口血流E峰、A峰及二者比值(E/A),比较两组之间以上各测值的差异。结果 A、B两组心脏超声检查结果其中舒张期室间隔厚度(IVSDd)、左室后壁厚度(LVPWDd)及左心室舒张功能指标E峰、A峰比值(E/A)三项指标差异具有统计学意义(P<0.05)。结论高血压患者脉压差增大与左心肥厚及左心室舒张功能减退密切相关。 Objective To investigate the correlation of pulse pressure of hypertensive patients with the left ventricular structure and function. Methods All 277 eases of hypertension were divided into pulse pressure(PP) ≥40mmHg in the Group A and 20mmHg ≤ PP≤40mmHg in the Group B , and cardiac ultrasound was used to detect and record the diastolic interventrieular septum thickness ( IVSDd), left ventricular posterior wall thickness (LVPWDd), left ventricular diastolic diameter (LVDd), systolic diameter (LVSd), left ventricular systolic function index election fraction(LVEF), left ventricular diastolic function mitral blood flow peaks E and A and E / A ratio in both groups . The above measured values between the two groups were compared. Results There were significant differences in IVSDd, LVPWDd and E/A ratio between Groups A and B(P〈0.05) Conclusion The increased pulse pressure in hypertension is closely related to left ventricular hypertrophy and left ventricular diastolic dysfunction.
