摘要
目的通过对各地疼痛科医生的调查，分析疼痛科医生素质对疾病正确诊断的影响。方法本调查采用横断面设计。从我院疼痛科所有住院病历中随机抽取1例，进行资料整理分为两部分，将两部分资料和调查表（分两个步骤进行）采用信件和电子邮件通讯方式对疼痛科医生进行调查。记录医生的相关资料并纳人多因素Logistic回归模型进行统计分析。结果共发出120份调查表，收回有效调查表98份，诊断正确64人。多因素Logistic回归分析显示职称、工作时间及疼痛科所在医院级别可以影响疾病诊断，其中主治医师以上职称、工作时间大于2年、有疼痛门诊及病房的医生诊断准确率较高。调查显示医生进行疾病诊断的主要依据是病史采集、体格检查、辅助检查以及医生的自身素质，薄弱环节为医生的知识结构；误诊主要在于病史采集。结论疼痛科医生的工作时间、职称及有无疼痛门诊及病房可以影响疾病能否正确诊断。
Objective To determine the influence factors on disease diagnosis by investigating the nationwide doctors in the pain department. Methods The cross-sectional study was designed. One sample was selected at random from the records of the all patients in the pain ward. The basic data from this sample was collected and sorted into two parts without diagnosis. A questionnaire was made with the history and was sent by post correspondence or email to 120 doctors nationwide who involved in pain management. The data on the doctors were recorded. The multiple logistic regres- sion was used to perform the statistical analysis. Results A total of 120 questionnaires was deliv ered, and 98 responses were received,in which diagnosis was correct in 64 responses. The multiple logistic regression analysis showed that there were three factors influencing the disease diagnosis： professional title, duration of employment, and with or without pain clinic or ward in the hospital. The doctors with professional title of attendant doctor or above, with employment duration more than 2 years, presence of the pain clinic or ward had higher correct diagnosis rate. It was considered that the main factors affecting correct diagnosis were disease history taking, physical examination, labora- tory examination, and the doctor＇s quality, and their weak points was knowledge structure related pain medicine. Conclusions The analysis of the data on the doctors in the pain department indicates that there are three factors that can affect the doctors ~ diagnosis to the pain diseases, including the doctors＇ employment duration, professional title, and presence of the pain clinic or ward in their hospital.
