摘要
目的探讨脑胶质瘤在二维质子磁共振波谱(two dimension proton magnetic resonance spectroscopy,2D1H-MRS)上的代谢特点及相对定量分析。方法收集经手术病理证实的脑胶质瘤患者28例。所有受试者术前均进行二维多体素1H-MRS扫描。测量脑胶质瘤患者病灶实质部分、瘤周水肿区、健侧相应部位的Cho/Cr、Cho/NAA、NAA/Cr、Lac/Cr、Lipid/Cr比值,并与对照组相应区域各比值比较。结果所有胶质瘤患者,肿瘤瘤体区的波谱谱线均表现为Cho升高,NAA、Cr明显下降,部分患者还出现Lac、Lipid峰;肿瘤周边区亦出现类似的异常谱线。肿瘤实质部与瘤周、肿瘤实质部与健侧组织Cho/Cr、Cho/NAA差异有统计学意义(P<0.001)。肿瘤实质部、瘤周与健侧组织比较NAA/Cr显著降低,差别有统计学意义(P<0.001),肿瘤实质部、瘤周与健侧组织比较Lac/Cr升高,差别有统计学意义(P<0.05)。肿瘤实质部与健侧Lipid/Cr比较差别有统计学意义(P<0.05),而肿瘤周围与肿瘤实质、肿瘤周围与健侧比较Lipid/Cr差别均无统计学意义(P>0.05)。结论二维多体素质子磁共振波谱1H-MRS能够较可靠反映脑胶质瘤代谢特点及浸润生长的特性,为胶质瘤诊断及鉴别诊断提供了一条新途径。
Objective To discuss the metabolic characteristics and relative quantitative analysis of cerebral gliomas on two dimension multiple pixels proton magnetic resonance spectroscopy(2D 1H-MRS).Methods Twenty-eight patients with cerebral gliomas confirmed by pathology were collected.All the patients received this examination.Regions of interesting(ROI) were placed in different places which included tumor portion,the place around the tumor and contralateral normal brain tissues.The metablotic ratios include Cho/Cr,Cho/NAA,NAA/Cr,Lac/Cr,Lipid/Cr were calculated and evaluated.Results All the patients showed the pathologic spectroscopy with abnormal high choline peak,decreased NAA peak and creatine peak,and occasional Lac and lipid peaks.There were similar metablotic manifestation of spectroscopy in some peritumor regions.The ratios of Cho/Cr and Cho/NAA significantly increased in the lesions compared with the peritumor regions and contralateral healthy areas(P 0.001);the metabolic ratio of NAA/Cr was found notable lower in the portion of leision and peritumor areas than in contralatera(P 〈0.001),whereas the ratio of Lac/Cr in the portion of leision and peritumor areas was higher than in contralatera(P 〈0.05).The metabolic ratio of Lipid/Cr increased in the lesion compared with contralateral(P 〈0.05).There was no significant difference in metabolic ratio of Lipid/Cr between peritumor region and leision region or between pertumor region and contralateral(P 〉0.05).Conclusion The metabolic properties and infiltrating growing characteristic of cerebral gliomas might be distinguished with multivoxel 2D 1H-MRS,which provides a new approach for diagnostic and differential diagnostic of brain gliomas.
