摘要
本文总结４３例二尖瓣腱索断裂的诊断和外科治疗经验。全组病例均有不同程度的心功能不全，２６例行二尖瓣置换术（ＭＶＲ），１７例行瓣膜修复成形术。术后存活３９例，４例围手术期死亡（死亡率９．３％）。存活病例术后随访２～５７个月。除其中１例风湿性病变患者行二尖瓣、三尖瓣成形术，术后１年又出现二尖瓣中度关闭不全外，其余心功能均有不同程度改善。我们初步认为风湿性病变及心内膜炎患者以ＭＶＲ为宜，退行性病变及先心病者应根据病理类型选择手术方式。
Fourty three cases suffered from mitral valve with ruptured chordae tendience were reported.All patients had different degree of heart failure.MVR was performed in 26 cases and MVP, in 17cases.39 cases were alive,and 4 cases died of operation.Mortality of all cases was 9.3%. Postoperative follow-up is 2～57 months in all patients alive.Heart function of patients alive was significantly improved.But one patient suffered from rheumatic heart disease who was treated with MVP and TVP recurred mild mitral valve regurgitation one year after operation.Conclusion:Patients suffered from rheumatic heart disease and endocarditis should treated with MVR,and Patients with congenital heart disease and degenerate heart disease may treat with MVP according to types of pathology.
