无创双水平正压通气治疗急性心源性肺水肿20例 被引量:6

Clinical Study of Noninvasive Bi-level Positive Airway Pressure Ventilation in Treatment of Acute Cardiogenic Pulmonary Edema

摘要 目的:探讨无创双水平正压通气(BIPAP)在急性心源性肺水肿的治疗效果。方法:急性左心衰患者20例同时接受常规抗心衰药物治疗,分别观察应用BIPAP 2h后的血气及心率等参数的变化,并作统计学分析。结果:20例患者经治疗后2h呼吸频率(RR)、心率(HR)显著减慢(P<0.05),呼吸困难缓解。血气提示PaO2和SaO2均显著提高(P<0.05),PaCO2显著降低(P<0.05),除1例死于原发性心室纤颤外,其余心悸、气促、呼吸困难症状均有明显改善。结论:在严格掌握应用指征的前提下,面罩式BIPAP用于治疗急性心源性肺水肿是有效的和安全的。 Objective:To assess the efficacy of noninvasive bi-level positive airway pressure(BiPAP) ventilation in patients with acute cardiogenic pulmonary edema.Methods:20 patients were treated with BiPAP for 2 hours as well as routine drugs used in the treatment of congestive heart failure.The variations of parameters such as clinical symptoms and arterial blood gas were recorded to evaluate the efficacy of BiPAP ventilation.Results:After 2 hours of noninvasive ventilation,We observed a statistically significant improvement in clinical symptoms,heart rate,PaCO2,PaO2 and arterial oxygen saturation (P<0.05).Conclusions:Noninvasive BiPAP ventilation can improve symptoms and hypoxemia of patients with acute pulmonary edema.It is a feasible and effective method for acute pulmonary edema.
