后入路复位侧块钉板/棒系统内固定术治疗颈椎骨折脱位

摘要 目的总结颈椎骨折脱位后入路复位侧块钉板/棒系统内固定术治疗的临床经验。方法回顾性分析自2004年至今,采用后入路复位外侧块钉板/棒系统内固定术治疗颈椎骨折脱位16例。结果所有病例颈椎生理曲度恢复良好,术后神经功能恢复有效率75%,植骨融合率100%。神经根损伤1例,无医源性脊髓及椎动脉损伤、术后感染及内固定物失效等并发症。结论后入路复位颈椎侧块钉板/棒系统内固定术治疗颈椎骨折脱位,既可从后方进行椎板减压,又能进行有效的复位及内固定,尤其适用于存在小关节突交锁的患者,是治疗颈椎骨折脱位的首选方案。 [Objective] To summarize the clinical experience of treatment on cervical vertebra fracture and dislocation with posterior reduction and lateral mass screw fixation.[Method]Retrospectively analyzed 16 cases of cervical vertebra fracture and dislocation treated by posterior reduction and lateral mass screw fixation,from 2004 to present. [Results]The cervical physiological lordosis recovered well in all cases, effective rate of nerve function recovery after operation was 75%, the fusion rate of bone graft was 100%, iatrogenic nerve root injury occurred in one case. there was no iatrogenic spinal cord or vertebral artery injury, no infection or internal fixation failure. [Conclusion]The procedure of posterior reduction and lateral mass screw fixation is not only available for posterior decompression,but also shows effective reduction and rigid internal fixation.We recommend it as the first choice for fracture and dislocation of cervical vertebra, especially the small zygopophysis interlocking cases.
