摘要
目的探讨经颅多普勒超声(transcranial Doppler ultrosonography,TCD)对脑动脉狭窄或闭塞的诊断价值。方法选择2008年3月~2009年3月在南华大学第一医院神经内科因患缺血性脑血管病住院行TCD及数字减影血管造影(digital subtraction angiography,DSA)检查的69例作为研究对象,将TCD与DSA的结果进行回顾性统计分析。根据病变血管不同将结果分为大脑中动脉(MCA)组、颈内动脉(ICA)组、椎动脉(VA)组、基底动脉(BA)组及锁骨下动脉(SUB-A)组。以DSA结果为诊断标准,对比TCD诊断各组血管狭窄或闭塞的特异性和敏感性,判定TCD诊断脑血管狭窄或闭塞的可靠程度,探讨脑血管狭窄或闭塞的合理检查程序。结果 TCD诊断MCA、ICA、VA、BA、SUB-A各组血管狭窄或闭塞的敏感性和特异性分别为83.33%、91.23%,56.52%、97.39%,76.19%、96.58%,93.33%、94.44%,88.89%、99.22%。TCD对MCA、BA、SUB-A狭窄或闭塞诊断的敏感性较高,对MCA、ICA、VA、BA、SUB-A狭窄或闭塞诊断的特异性较高。结论 TCD对脑动脉狭窄或闭塞是无创、可行和经济的筛查方法,可用于可疑病例的初步诊断;TCD与DSA之间有良好的相关性。
Objective To evaluate value of transcranial Doppler ultrosonography (TCD) in diagnosis of cerebral arteries stenosis or occlusion in patients with intra-or extra-cranial stenosis or occlusion.Methods 69 patients hospitalized during March 2008 and March 2009 for ischemic cerebrovascular disease were chosen as subjects of study.All the patients underwent TCD and digital subtraction aniography (DSA).The results were retrospectively analyzed.All the patients were divided into 5 groups:middle cerebral artery(MCA) group,internal carotid artery(ICA) group,basilar artery(BA) group,vertebral artery(VA) group,and subclavian artery(SUB-A) group.The result of DSA was set as diagnostic criteria and in comparison with the result of TCD,the specificity and sensibility of cerebral artery stenosis or occlusion of all the groups and reasonable checker of cerebral artery stenosis were discussed.Results Analysis showed that the sensibility and specificity of TCD in diagnosing the above groups were 83.33% and 91.23%,56.52% and 97.39%,76.19% and 96.58%,93.33% and 94.44%,88.89 % and 99.22% respectively.The sensibility of TCD in diagnosing MCA,BA,SUB-A and the specificity of TCD in diagnosing MCA,ICA,VA,BA,SUB-A turned out better results.Conclusion There should be some correlation between TCD and DSA in diagnosing cerebral arteries setnosis or occlusion.This mehtod porvides a noninvasive and relibale mehtod for diagnosing cerebral arteries stenosis or occlusion and can be used in diagnosing equivocal cases.
