胸椎椎弓根螺钉与椎弓根外侧螺钉固定治疗胸椎骨折的比较研究 被引量:5

Comparative Study on Thoracic Pedicle Fixation with Extrapedicle Fixation in Treatment of Thoracic Vertebrae Fractures

摘要 目的回顾性分析比较胸椎椎弓根螺钉与椎弓根外侧螺钉固定治疗胸椎骨折的临床疗效。方法将24例胸椎骨折分为A组和B组,A组为经传统的胸椎椎弓根螺钉置入固定,B组为经椎弓根外侧螺钉置入固定,回顾性比较两组患者术中手术持续时间、出血量、X线透视次数、胸椎椎弓根骨壁穿透率,应用Frankel分级对术前、术后的脊髓功能进行评价。结果 A、B两组患者手术持续时间、术中出血量、术中X线透视次数及椎弓根骨壁穿透率均有显著性差异(P<0.01),脊髓功能除A级7例无变化外,两组患者术后较术前均有1~2级的恢复,脊髓功能得到了较好的改善。结论胸椎椎弓根外侧螺钉固定治疗胸椎骨折具有手术时间缩短,出血量减少,手术更安全,术中对患者及术者的X线暴露量减少的优点,其疗效与传统胸椎椎弓根路径手术相近。 Objective To compare clinical outcomes of the thoracic pedicle screw fixation versus extrapedicular screw fixation in the treatment of thoracic vertebrae fractures.Methods Twenty-four patients with thoracic vertebrae fractures were treated by thoracic pedicle screw fixation or extrapedicular screw fixation and were randomly divided into two groups.In group A, thoracic pedicle screws were placed using traditionally pedicular approach bilaterally;while thoracic pedicle screws were placed using extrapedicular approach bilaterally in group B.The two groups were compared in terms of operation durational time, blood loss, radiography times and thoracic pedicle cortical perforation number. The function of spinal cord was evaluated before and after the operations with JOA scores and the improvement rate was calculated.Results Significant differences were observed between the two groups in operation durational time, blood loss, radiography times and thoracic pedicle cortical perforation number(P〈0.01).The significant differences in improvement rates between score before operation and score after operation were identified, but the improvement rate had no significant difference in the two types of operations. Conclusion Compared with the transpedicular fixation, the extrapedicular fixation has shorter operation time, less blood loss, safer and less radiography time. It has the same results with the traditional pedicular approach.
