关节镜微创治疗军事训练导致的髌股关节炎 被引量:3

摘要 目的评价关节镜微创治疗军事训练导致髌股关节炎的疗效。方法对84例髌股关节疾病患者按照Merchant分型进行分类后随机分为两组,非手术治疗组39例48膝,关节镜治疗组45例55膝。关节镜组镜下软骨修整、软骨下骨钻孔、游离体摘除。对两组膝关节功能恢复情况进行统计学比较。结果两组患者均获随访,时间1~6(3.8±0.4)年。按Ikeuchi膝关节评分标准进行功能评定:非手术治疗组优良率56.4(55.2±0.8)%,关节镜组优良率89.7(88.2±0.4)%,两组比较差异有统计学意义(P<0.05)。结论关节镜下清理术治疗军事训练导致的髌股关节炎具有创伤小、恢复快、疗效确切等优点。 Objective To evaluate the minimally invasive treatment of the arthroscopic debridement for the patellofemoral osteoarthritis resulting from military training.Methods 84 cases of patellofemoral disorders were randomized into two groups after being classified according to Merchant's classification.One group was treated conservatively.The other group was treated with arthroscopic debridement,including articular cartilage repairing,subchondral bone drilling,loose bodies removing.All cases were followed after operation and compared the restored function of each group with statistical methods.Results All the cases were followed for 1~6(3.8±0.4)years.The restored function of each case was estimated complying with the Ikeuchi standard.The average rate of excellent and good result in conservative group was 56.4(55.2±0.8)%.The average rate of that was 89.7(88.2±0.4)% in arthroscopic group.There were statistical differences(P0.05)between the results of conservative group and arthroscopic one.Conclusions Arthroscopic debridement is a good treatment choice to the patellofemoral osteoarthritis for its less injury and rapid recovery.
