摘要
目的探讨乙酰化血红蛋白对不同原理的高效液相色谱法检测血红蛋白A1c(HbA1c)的影响。方法用美国Primus公司出品的基于硼酸盐亲和层析法(PDQ系统)和由美国Bio-Rad公司出品的基于离子交换层析法的糖化血红蛋白检测系统(D-10系统)检测慢性肾功能不全患者40例,并对体外乙酰化标本进行检测。结果 D-10系统检测慢性肾功能不全结果高于PDQ系统,且差异有统计学意义(P<0.05),同时乙酰化血红蛋白干扰时,D-10系统的检测结果高于实际值,而PDQ系统的结果与实际值相符。结论血红蛋白乙酰化水平增高会影响D-10系统检测HbA1c,而PDQ则不受影响。
Objective To discuss the effect of Carbamyl-Hbs on the assay of glycohemoglobin HbA1c by high performance liquid chromatography (HPLC).Methods The result of hemoglobin HbA1c from 40 samples with chronic renal failure were obtained using the Primus PDQ(based on boronate-affinity chromatography) and the Bio-Rad D-10(based on ion-exchange chromatography);and a test of Carbamyl-Hb in vitro was designed in the assay of HbA1c by above-mentioned two HPLC.Results In the interference test,the results of HbA1c could be obtained acceptable by PDQ,but the results from D-10 exceeded the true,The results of the patients with chronic renal failure from D-10 system exceeded obviously the results from the PDQ system, and they showed statistically significant differences(P〈0.05)between two systems.Conclusion The capability of anti-interference to Carbamyl-Hbs of boronate affinity chromatography is excelled that of ion-exchange chromatography.
