摘要
目的观察曲安奈德联合爱维治治疗前部缺血性视神经病变(AION)早期患者的临床疗效。方法将临床确诊的AION患者54例(56只眼)随机分为2组,治疗组27例(28只眼)接受曲安奈德20 mg经Tenon囊球后注射,间隔14 d后重复1次,并用爱维治1200 mg每日1次静脉滴注;对照组27例(28只眼)仅采用爱维治1200 mg每日1次静脉滴注。疗程均为4周。用药后每天观察视力和眼底变化情况,治疗前有视野者复查视野。结果治疗4周后,治疗组视力高于0.5的眼数明显多于对照组,差异有统计学意义(P<0.01),动态视力观察治疗后第1、2、3、4周治疗组的视力提高程度均明显高于对照组,差异有统计学意义(P<0.05,P<0.01)。治疗组有视野检查的14只眼中治疗后好转12只眼(85.7%),对照组有视野检查的15只眼中治疗后视野好转6只眼(40.0%),2组视野变化比较差异有统计学意义(P<0.01)。治疗组视乳头水肿明显减轻或消退20只眼(71.4%),对照组12只眼(42.9%),2组比较差异有统计学意义(P<0.05)。结论曲安奈德联合爱维治治疗AION早期疗效优于单纯爱维治,能安全有效地促进视功能恢复。
Objective To observe the therapeutic effect of triamcinolone acetonide（TA） combined with actovegin on early stage of anterior ischemic opticneuropathy（AION）.Methods Totally 54 patients（56 eyes） diagnosed with AION were randomly divided into two groups,27 patients（28 eyes）underwent retrobulbar injection with triamcinolone acetonide（TA） combined intravenous drip with actovegin（1200 mg/d）（treatment group）,and the other 27 patients（28 eyes） underwent intravenous drip with actovegin（1200 mg/d）（control group）.The period of treatment of two groups were both 28d.The changes of visual acuity and ocular fundus of the patients after injection were recorded per day,and the visual field was counterchecked in the patients who had underwent the examination before the treatment.Results The visual acuity（〉0.5） was better in treatment group than that in control group with signi ficant difference（P〈0.01）.At the time of 1w,2w,3w and 4 w after treatment,the visual acuity was better in treatment group than that in control group with significant difference（P〈0.05,P〈0.01）.In 14 eyes in treatment group which had undergone examination of visual field,the visual field improved in 12（85.7%） eyes.In 15 eyes in control group which had undergone examination of visual field,the visual field improved in 6（40.0%） eyes.There was significant difference between the two groups（P〈0.01）.Conclusion The therapeutic effect of triamcinolone acetonide combined with actovegin on early stage AION was better than that of actovegin alone,and could improve visual function obviously.
